LAS VEGAS — If you have both Medicare and Nevada Medicaid, your plan choices for 2027 look a little different this fall. In Clark County — the county Las Vegas sits in — 16 Dual Special Needs Plans (D-SNPs) are being offered for the 2027 plan year, from six carriers, according to the CMS Medicare Advantage Landscape Source File. A few are new, a few have been renamed, and the rules for when a dual-eligible person can change plans are more flexible than the rules for everyone else.
The Annual Enrollment Period runs October 15 – December 7, 2026, and anything you choose during that window starts January 1, 2027. But if you are dual-eligible, you also have extra chances to switch during the year — more on that below.
What a D-SNP is, in plain words
A Dual Special Needs Plan is a type of Medicare Advantage plan built for people who have both Medicare and Medicaid. Every D-SNP includes drug coverage, and plans coordinate with your Medicaid benefits so that one plan is helping manage both sides of your care.
D-SNPs are not the only Special Needs Plans sold in Clark County for 2027 — there are also 16 Chronic Condition SNPs (C-SNPs) and 1 Institutional SNP (I-SNP) — but those have different eligibility rules.
Who qualifies: full vs. partial dual-eligible
"Dual-eligible" is an umbrella term. Nevada Medicaid (through the state's Division of Welfare and Supportive Services) is who decides which category you fall into:
- Full dual-eligible: You have Medicare and full Nevada Medicaid benefits. Medicaid helps with services Medicare does not cover (such as long-term services and supports) and typically pays your Medicare cost-sharing.
- Partial dual-eligible: You qualify for a Medicare Savings Program (QMB, SLMB or QI) that helps pay Medicare premiums and, in some cases, cost-sharing — but you do not have full Medicaid benefits.
Some D-SNPs are designed for people with full Medicaid, and some are designed for partial dual-eligibility. The plan name sometimes hints at this (you'll see words like "Full Dual" or "Partial Dual"), but the plan's Evidence of Coverage and the Medicare Plan Finder at medicare.gov will tell you exactly who each plan accepts.

A renamed plan is not necessarily the same plan with a new label — the Annual Notice of Change (ANOC) your carrier sent in September will spell out what, if anything, is different for 2027.
How often a dual-eligible person can change plans
If you only have Medicare, you generally pick a plan during the Annual Enrollment Period (Oct. 15 – Dec. 7) and, if you're already in a Medicare Advantage plan, you have one more chance to switch during the Medicare Advantage Open Enrollment Period (Jan. 1 – Mar. 31).
Dual-eligible people — and anyone receiving Extra Help with Part D — have more flexibility. Under current CMS rules, you generally get a monthly opportunity to:
- Enroll in an integrated D-SNP that works with your Medicaid plan, or
- Leave a Medicare Advantage plan and return to Original Medicare with a stand-alone Part D drug plan.
The specific month-to-month rules have changed over the past couple of years, and they can change again. Before you make a switch, call 1-800-MEDICARE (TTY 1-877-486-2048) and confirm which Special Enrollment Period applies to your situation.
How to confirm your eligibility
Two things to check before you enroll:
1. Your Medicaid status. Contact Nevada Medicaid through Access Nevada (accessnevada.dwss.nv.gov) or your local Division of Welfare office. They can tell you whether you have full Medicaid, a Medicare Savings Program, or both. 2. Your Medicare status. Log in at medicare.gov or call 1-800-MEDICARE. You can see your current plan, your Extra Help status, and every plan available in your ZIP code side by side.
If your Medicaid status changes during the year — for example, you lose it or move from partial to full — your D-SNP may need to end, and you'll get a Special Enrollment Period to pick a new plan.
What else is changing for 2027
A few program-level numbers to keep in mind as you compare plans:
- The Part D out-of-pocket cap rises to $2,400 in 2027 (up from $2,100 in 2026).
- The Part D maximum deductible rises to $700 in 2027 (up from $615 in 2026).
- The 2027 Part B standard premium, Part B deductible, and IRMAA income brackets have not been announced yet. CMS typically publishes them in the fall.
