What’s Changing in New Hampshire Medicare Advantage for 2027: 8 Plans Leaving, 3 Changing
CONCORD, N.H. — The federal government has published the 2027 Medicare Advantage Landscape files, and the picture for New Hampshire has come into focus. According to a side-by-side read of the CMS Landscape Source Files for CY2026 and CY2027, the number of Medicare Advantage plans offered somewhere in the state will tick up from 19 for 2026 to 20 for 2027. Behind that one-plan difference is a much bigger reshuffle: 8 plans are leaving, 9 are new, 3 are changing in at least one county, and 8 are continuing unchanged.

Key takeaways
- 8 Medicare Advantage plans are leaving New Hampshire for 2027 and 3 are changing in at least one county, so every member should open their Annual Notice of Change when it arrives.
- If your plan is not renewed, you have a Special Enrollment Period that runs through the end of February 2027 to pick a new plan.
- The Annual Enrollment Period runs October 15 through December 7, 2026, and the choice you make takes effect January 1, 2027.
- Free, unbiased help is available from Medicare.gov, 1-800-MEDICARE, and New Hampshire's State Health Insurance Assistance Program (SHIP).
Every one of New Hampshire's 10 counties will see at least one plan leave or change. Two carriers — Aetna Medicare and WellSense Health Plan — have no New Hampshire plans listed for 2027. One carrier, Devoted Health, is new to New Hampshire for 2027.
Here is what the Landscape files say, what the rules say, and what a Granite State member can do between now and December 7, 2026.
What the statewide numbers mean
CMS publishes a Landscape Source File each fall that lists every Medicare Advantage plan offered in every county, with its premium, drug deductible, plan type and plan name. When analysts compare one year's file to the next, "leaving" means a plan that was offered somewhere in New Hampshire for 2026 and nowhere in the state for 2027. "Changing" means a plan that appears in both years' files but differs on premium, drug deductible, plan type or plan name in at least one county. "New" means a plan listed for 2027 that was not in the 2026 file. "Continuing unchanged" means those four fields match in every county the plan serves.
For New Hampshire, counting each plan once statewide no matter how many counties it serves, the 2026-to-2027 comparison shows: 19 plans for 2026, 20 for 2027, with 8 leaving, 9 new, 3 changing and 8 continuing unchanged. Aetna Medicare and WellSense Health Plan have no New Hampshire plans in the 2027 file. Devoted Health appears in New Hampshire for the first time for 2027.
If your plan is leaving
If your plan is one of the 8 leaving, your carrier is required to send you a non-renewal notice. You do not lose Medicare — Parts A and B continue — but your Medicare Advantage plan will end on December 31, 2026.
Two timelines are open to you. You can use the Annual Enrollment Period (October 15 – December 7, 2026) to pick a new Medicare Advantage plan or return to Original Medicare, with a choice that takes effect January 1, 2027. You also get a Special Enrollment Period that runs through the end of February 2027 specifically because your plan was not renewed, giving you extra time to choose.
If you do nothing, you will not be automatically enrolled in a new Medicare Advantage plan. You will generally return to Original Medicare on January 1, 2027, without a Part D drug plan unless you actively elect one.
If your plan is changing
Three plans are offered in both 2026 and 2027 but differ in the 2027 Landscape file on premium, drug deductible, plan type or plan name in at least one county. If you are in one of these plans, the specifics for your county are in your Annual Notice of Change (ANOC), the document your carrier mails each fall before the Annual Enrollment Period.
The ANOC is the authoritative, member-specific summary of what is different for the coming year. Read it next to your current member materials, and pay attention to any section that compares this year to next year. If you do nothing, you stay in the plan with the changes described in the ANOC.
Counties with the most change
Based on the county-level comparison in the Landscape files, every one of New Hampshire's 10 counties sees at least one plan leave or change for 2027. The counts, by county:
- Hillsborough County: 8 leaving, 3 changing
- Rockingham County: 5 leaving, 3 changing
- Merrimack County: 3 leaving, 3 changing
- Strafford County: 3 leaving, 3 changing
- Cheshire County: 3 leaving, 0 changing
- Grafton County: 2 leaving, 1 changing
- Sullivan County: 2 leaving, 1 changing
- Belknap County: 1 leaving, 1 changing
A plan that is leaving statewide shows up as "leaving" in every county where it was offered for 2026, which is why the county totals add up to more than the statewide figure of 8.
What to do before December 7
A short checklist for the weeks ahead:
1. Watch your mail in September and October for the ANOC from your current plan, or a non-renewal notice if your plan is one of the 8 leaving. 2. Make a list of the doctors, hospitals and prescriptions you need to keep. 3. Between October 15 and December 7, 2026, compare plans available in your county on Medicare.gov's Plan Finder, which uses the same CMS Landscape and Plan Finder data analysts use for year-over-year comparisons. 4. If you want to enroll, switch or drop a plan, do it by December 7, 2026 so coverage starts January 1, 2027. If your plan is not renewed, your Special Enrollment Period extends through the end of February 2027.
