What a C-SNP is and who qualifies

A Chronic Condition Special Needs Plan is a type of Medicare Advantage plan limited to people living with one or more qualifying chronic conditions defined by Medicare. Common examples include diabetes, chronic heart failure, and chronic lung disorders, but the specific condition that a given C-SNP covers is set by that plan.

To enroll, a health care provider must verify that you have the qualifying condition the plan is built around. If that verification does not come through, the plan cannot keep you enrolled. Like other Medicare Advantage plans, C-SNPs bundle Part A and Part B coverage, and the ones listed in New York for 2027 all include drug coverage.
