NEW YORK CITY — October 2026. If you have a Medicare Advantage plan in New York City, this is the fall when next year's lineup comes into focus. In New York County, the county that covers Manhattan, 20 Medicare Advantage plans will not return for 2027, six new plans are joining the market, and one carrier — Blue Cross Blue Shield (HCSC) — will no longer offer a plan here. Fifty-three plans continue, and most of them have at least one change on paper worth reading before you renew.

The Annual Enrollment Period runs October 15 through December 7, 2026, and any choice you make in that window takes effect January 1, 2027. If you do nothing and your current plan is still offered, it renews automatically with the updates spelled out in your plan's Annual Notice of Change (ANOC).

The 2027 lineup at a glance

New York City is in New York County, where Medicare Advantage availability is set by CMS. For 2026, 16 carriers offer 75 Medicare Advantage plans in the county — 39 HMOs, 20 HMO-POS plans and 16 PPOs. Forty of those are Special Needs Plans (SNPs) for people who are dually eligible for Medicare and Medicaid, live in a nursing home, or have certain chronic conditions. All 75 include prescription drug coverage.

For 2027, the carrier roster shrinks by one as Blue Cross Blue Shield (HCSC) exits the county. The remaining carriers — listed alphabetically — are Aetna (CVS Health), Centene (WellCare), Elderplan, ElderServe Health, Elevance Health (Anthem), EmblemHealth, Hamaspik, Healthfirst Medicare Plan, Humana, Longevity Health Plan, MetroPlus Health Plan, Senior Whole Health of New York, UnitedHealthcare, VillageCareMAX and VNS Health Medicare.

What's new for 2027

Six Medicare Advantage plans are new to New York County for 2027. In alphabetical order by carrier:

Note that some of these names echo plans leaving the market — the plan numbers (what CMS calls the contract and plan ID) are different, so these are new offerings even when the branding looks familiar. Always confirm the plan ID in Medicare's Plan Finder before you enroll.

Two plans have been renamed but continue: Wellcare Fidelis Dual Align Unity (HMO D-SNP), formerly Wellcare Fidelis Dual Align (HMO D-SNP); and UHC Dual Complete NY-YL (HMO-POS D-SNP), formerly UHC Dual Complete NY-Y001 (HMO-POS D-SNP).

Plans not returning after 2026

Twenty Medicare Advantage plans are leaving the New York County market at the end of 2026. If you are enrolled in one of these, you'll receive a non-renewal notice from your carrier.

If your plan is on this list, you qualify for a Special Enrollment Period that lets you pick a new Medicare Advantage plan or move to Original Medicare (with or without a stand-alone Part D plan). The SEP for a non-renewed plan generally runs from December 8, 2026 through the end of February 2027, but acting during the Annual Enrollment Period keeps your coverage continuous on January 1.

What's changing on continuing plans

Fifty-three plans are continuing into 2027. CMS data shows 11 of them have a change on at least one of these fields: premium, deductible, plan type or name. That doesn't mean your out-of-pocket costs necessarily move in one direction — only that something in the plan's basic structure is different.

The practical step is the same for every renewing enrollee: read the Annual Notice of Change your plan mails in September or early October. It summarizes what's different for 2027, including any changes to the drug formulary, the provider network, and the extra benefits.

Program-level numbers for 2027

A few Medicare rules change for everyone with Part D next year, regardless of which plan you pick:

The 2027 Part B standard premium and deductible and the 2027 IRMAA income brackets have not been announced yet. CMS typically publishes those figures later in the fall. We'll update as soon as they are released.

How to use this during Annual Enrollment

A short checklist for New York City readers:

1. Open your ANOC. If your plan is continuing, this one document tells you what's different for 2027. 2. Confirm your doctors and your drugs. Networks and formularies can shift from year to year. Medicare's Plan Finder at medicare.gov lets you plug in your prescriptions and see how each plan would cover them. 3. If your plan is leaving, you have time. You can pick a new plan during AEP (by December 7) or during your Special Enrollment Period, but choosing by December 7 keeps your coverage seamless. 4. Think about plan type, not just name. HMOs generally require you to stay in network and use a primary care doctor; HMO-POS plans allow some out-of-network access; PPOs offer the most flexibility. A Special Needs Plan is only an option if you meet its eligibility rules.
