- A C-SNP is a Medicare Advantage plan limited to people with a specific chronic condition, and enrollment requires a provider to confirm the qualifying diagnosis.

- Every C-SNP in Dallas County for 2027 includes Part D drug coverage; the Part D out-of-pocket cap is $2,400 in 2027 ($2,100 in 2026).
DALLAS — As the Annual Enrollment Period opens on October 15, 2026, people in Dallas County who live with a serious chronic condition have a specific slice of Medicare Advantage to look at: Chronic Condition Special Needs Plans, or C-SNPs. For 2027, 15 of the 84 Medicare Advantage plans offered in Dallas County are C-SNPs, built around a qualifying diagnosis and the care that goes with it.
What a C-SNP is, in plain language
A Chronic Condition Special Needs Plan is a type of Medicare Advantage plan designed for people with one of the chronic conditions CMS has approved for Special Needs status — for example, diabetes, chronic heart failure or end-stage renal disease. The plan's provider network, drug list, care coordination and extra benefits are shaped around that condition.
Because a C-SNP is a Medicare Advantage plan, it replaces Original Medicare for the people enrolled in it. Every C-SNP offered in Dallas County for 2027 includes Part D prescription drug coverage, so a separate drug plan is not needed.
Who qualifies, and how enrollment works
To join a C-SNP, you need the qualifying chronic condition the plan is built for, and your doctor or other provider has to confirm the diagnosis. The plan will ask for that verification — usually a short form your provider signs — within a set window after you apply. If the diagnosis cannot be confirmed, the plan cannot keep you enrolled.
Having a qualifying condition also gives you a Special Enrollment Period. That means you can enroll in a C-SNP outside the usual Annual Enrollment Period once you have the diagnosis, instead of waiting until next fall.
The C-SNPs offered in Dallas County for 2027
Six carriers offer C-SNPs in Dallas County next year: Aetna (CVS Health) with 1, Elevance Health (Anthem) with 4, Humana with 5, ProCare Advantage with 2, UnitedHealthcare with 2, and Verda Health Plan of Texas with 1. The table below lists every C-SNP by carrier and plan type.
A few names tell you the condition they are built for: Wellpoint Kidney Care, Humana DaVita Kidney Care, Humana Gold Plus - Diabetes and Heart, ProCare Advantage - Diabetes Care Management and ProCare Advantage - Kidney Care all state their focus in the plan name. For every other C-SNP — including Aetna Medicare Chronic Care Total, the Wellpoint Chronic Care plans, UHC Complete Care TX-29 and TX-3P, and Verda Noble Chronic Care — check the plan's own documents on medicare.gov to see which qualifying conditions it covers.
What's new, renamed or returning for 2027
The C-SNP lineup in Dallas County is shifting a bit for 2027. New for next year are two Humana DaVita Kidney Care (PPO C-SNP) plans, an additional Humana Gold Plus - Diabetes and Heart (HMO C-SNP), Wellpoint Chronic Care Advantage Select (HMO C-SNP) and Wellpoint Chronic Care Select (HMO-POS C-SNP). Aetna Medicare Chronic Care Total (HMO C-SNP) is the renamed version of what was Aetna Medicare Prime Chronic Total (HMO C-SNP).
If you are already in a C-SNP, your Annual Notice of Change (ANOC) — the booklet your plan mails in September — spells out what is different for 2027 in your plan specifically.
Dates and figures to keep in mind
The Annual Enrollment Period runs October 15 – December 7, 2026, and any change made in that window takes effect January 1, 2027. If you do nothing, your current plan renews for 2027 as long as it is being offered, with the changes listed in your ANOC.
A few confirmed 2027 program numbers to anchor your planning: the Part D out-of-pocket cap rises to $2,400 in 2027 (from $2,100 in 2026), and the Part D maximum deductible rises to $700 in 2027 (from $615 in 2026). CMS publishes the 2027 Part B standard premium, Part B deductible and IRMAA income brackets later in the fall — typically in November — so those are not final yet.
How to compare your choices
Because C-SNP networks, drug lists and extra benefits are built around a specific condition, the most useful comparison is the one that uses your diagnosis, your doctors and your medications. The Medicare Plan Finder at medicare.gov lets you enter all three and see each plan's specifics side by side. You can also call 1-800-MEDICARE (TTY 1-877-486-2048), 24 hours a day, 7 days a week. For the fine print on a particular plan, the Evidence of Coverage and provider and pharmacy directories on the carrier's site are the definitive sources.
