MyMedicarePlans.org is an independent consumer news and education site. We are not Humana, and we are not Medicare. Humana is one of 10 carriers selling Medicare Advantage in Dane County for 2027; together, those carriers offer 48 plans. This article focuses on Humana because that is what the headline promises — not because we are steering you toward it.
How many Humana Medicare Advantage plans are offered in Madison for 2027?
In Dane County, which is where Madison sits, Humana is offering 12 Medicare Advantage plans for the 2027 plan year. By plan type, that breaks down to 2 HMO plans, 9 PPO plans (including two Regional PPOs) and 1 Private Fee-for-Service (PFFS) plan. Nine of the 12 include Part D prescription drug coverage; three do not.
Medicare Advantage availability is set at the county level, not the city level, so these counts apply to everyone with a Dane County ZIP code, not just people inside Madison's city limits. The full plan list, exactly as Humana files it with CMS, is in the table below.
What is new, renamed or not returning from Humana in Dane County for 2027?
Three things are changing in Humana's Dane County lineup for 2027. One plan is new: HumanaChoice SNP-DE H5216-420 (PPO D-SNP), a Dual Eligible Special Needs Plan with drug coverage. One plan from 2026 is not returning. And one plan has been renamed — Humana Total Complete (HMO) is the 2027 name for what was called Humana Gold Plus H6622-001 (HMO) in 2026.
A rename is not the same as a brand-new plan, but it can come with benefit, network or cost changes. The Annual Notice of Change (ANOC) your plan sent in late September is the document that lays out, in writing, what is different for 2027. If you cannot find yours, request a copy from Humana or look it up on medicare.gov.
Which Humana plans in Madison are Special Needs Plans, and who are they for?
Humana is offering 2 Special Needs Plans in Dane County for 2027, both of them D-SNPs (Dual Eligible Special Needs Plans). D-SNPs are built for people who qualify for both Medicare and Medicaid. There are no Chronic Condition SNPs (C-SNPs) or Institutional SNPs (I-SNPs) in Humana's Dane County lineup for 2027.
If you are not dually eligible, you are not eligible to enroll in a D-SNP, even if the plan looks appealing on paper. If you think you might qualify for Medicaid in Wisconsin, the Wisconsin Department of Health Services can confirm your status; Medicare.gov can then show which D-SNPs are open to you.
How do I check a Humana plan against my doctors and prescriptions before Dec. 7?
The fastest way is the Medicare Plan Finder at medicare.gov. You can enter your ZIP code, add your prescriptions and dosages, and add your preferred pharmacies; the tool then shows how each plan handles those specific drugs, including whether the plan uses the Medicare Prescription Payment Plan (the optional monthly smoothing of out-of-pocket drug costs that became available in 2025).
For doctors and hospitals, do not rely only on a plan's online directory — call the provider's billing office and ask whether they are in-network for the specific Humana plan you are considering for 2027, by its exact name and plan ID. Networks can differ sharply from one Humana plan to another. If you prefer to talk to a human, 1-800-MEDICARE (TTY 1-877-486-2048) is available 24 hours a day, seven days a week.
What program-level numbers should I know for 2027?
Two Part D figures for 2027 are already confirmed by CMS: the annual out-of-pocket cap for prescription drugs is $2,400 (up from $2,100 in 2026), and the maximum Part D deductible a plan can charge is $700 (up from $615 in 2026). The coverage gap, often called the donut hole, remains eliminated — Part D has had three phases, not four, since 2025.
Two other 2027 figures are not out yet. CMS normally announces the standard Part B premium and deductible, along with the IRMAA (income-related) surcharge brackets, in the fall — usually in November. Until that announcement, treat any 2027 Part B number you see online as an estimate, not a fact.
What happens if I do nothing by Dec. 7, 2026?
If you take no action during the Oct. 15 – Dec. 7, 2026 window, your current Medicare Advantage plan simply renews for 2027, as long as the carrier is offering it again. The changes described in your ANOC — to premiums, copays, drug tiers, networks or extra benefits — take effect Jan. 1, 2027.
If your current plan is not being offered again next year, Humana (or your current carrier) is required to notify you in writing. You then have a Special Enrollment Period to choose a replacement. And even after Dec. 7, Medicare Advantage members have one more chance to switch during the Medicare Advantage Open Enrollment Period, Jan. 1 – March 31, 2027.
