- Extra Help is a federal program that can lower Part D premiums, deductibles, and copays for people who qualify, and it isn't tied to any one insurance company or plan.
- The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027, and the maximum Part D deductible is $615 in 2026 and $700 in 2027. Extra Help can reduce those costs even further.
- Thanks to the Inflation Reduction Act, people with income at or below 150% of the federal poverty level can now get full Extra Help benefits, as long as their resources stay under the program limits.
- You can apply for free anytime online at ssa.gov/extrahelp, by phone at 1-800-772-1213, or in person at a Social Security office, without needing to pick a Part D plan first.
What Extra Help Actually Does
Extra Help, also known as the Low-Income Subsidy (LIS), is a Medicare program that helps pay for Part D prescription drug costs. That includes your monthly plan premium, the yearly deductible, and the copays at the pharmacy counter.
The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027, and the maximum Part D deductible is $615 in 2026 and $700 in 2027. Extra Help can reduce those costs further for people who qualify — in some cases, drug copays drop to just a few dollars per prescription.
The program is run by the Social Security Administration and the Centers for Medicare & Medicaid Services (CMS). It is not tied to any one insurance company or plan.
Who Qualifies in 2027
To be eligible, you must:
- Have Medicare Part A and/or Part B,
- Live in one of the 50 states or D.C., and
- Have income and resources below limits set by Social Security each year.
Thanks to changes from the Inflation Reduction Act, Extra Help now offers full benefits to people whose income is at or below 150% of the federal poverty level, as long as their resources stay under program limits. The partial benefit tier that used to exist has been folded into the full benefit.
Some people are enrolled automatically. If you have Medicaid, get help from a Medicare Savings Program, or receive Supplemental Security Income (SSI), you should already qualify — no application needed. Social Security will mail you a notice.
What Counts as Income and Resources
Income includes wages, Social Security payments, pensions, and similar money coming in each month.
Resources means things like money in checking and savings accounts, stocks, and bonds. It does not count your home, your car, personal belongings, burial plots, or a modest amount set aside for burial expenses.
Because the limits change slightly each year, always check the current numbers at SSA.gov/extrahelp before assuming you don't qualify. Many people are surprised to learn they're within the limits.
How to Apply
There are three easy ways to apply — and it's free:
1. Online at ssa.gov/extrahelp. 2. By phone at 1-800-772-1213 (Social Security). 3. In person at your local Social Security office.
The application is short compared to many government forms. You do not need to have your Part D plan picked out first — you can apply for Extra Help any time of year.
Emporia-Area Help Is Available
Emporia is in Lyon County, where residents have access to a range of stand-alone Part D drug plans and Medicare Advantage plans that include drug coverage. Extra Help works with either type, so qualifying doesn't lock you into one path.
For free, unbiased, one-on-one help, Kansas has a State Health Insurance Assistance Program (SHIP) called Senior Health Insurance Counseling for Kansas (SHICK). Trained counselors can help you check eligibility, fill out the application, and compare drug plans. Call SHICK at 1-800-860-5260.
What Happens After You Qualify
Once you're approved, Social Security sends a letter explaining your benefit. If you don't already have a Part D plan, Medicare will help enroll you in one so your coverage can start.
People with Extra Help also get a Special Enrollment Period, meaning you can change your drug plan more often than once a year if your needs change. That's especially useful if a medication changes or you move.
You can also look into the new Medicare Prescription Payment Plan, which lets anyone with Part D spread their drug costs across the year in monthly payments instead of paying big amounts at the pharmacy counter. It's separate from Extra Help, but the two can work together.
