Extra Help in Lancaster: the Medicare drug program that resets what you pay at the pharmacy

Lancaster, Texas — If paying for prescriptions each month feels like a stretch, there is a federal program worth knowing about before you finalize your 2027 Medicare coverage this fall. It is called the Low-Income Subsidy, better known as Extra Help, and it changes what you pay for Part D drugs — the premium, the deductible and the cost at the pharmacy counter. It is run by Medicare and the Social Security Administration, and in recent years Congress expanded it so that more people now qualify at the full benefit level.

What is Extra Help and who is it for in Lancaster?

Extra Help is the federal Low-Income Subsidy that reduces Medicare Part D drug costs for people with limited income and savings. It is for anyone with Medicare who also meets the income and resource limits set by the Social Security Administration — including people in Lancaster who have Medicare only, and people who have Medicare together with Medicaid.

If you qualify, Extra Help can reduce or eliminate your Part D monthly premium (up to a benchmark amount), lower or remove the plan deductible, and cap what you pay for each covered prescription at the pharmacy. People who have both Medicare and full Medicaid, who get Supplemental Security Income, or who are in a Medicare Savings Program are automatically enrolled — you do not have to apply.

Who qualifies for Extra Help in 2026?

To qualify for Extra Help, your income and resources must fall under limits that the Social Security Administration updates each year. As of 2024, Congress eliminated the old "partial" Extra Help tier, so everyone who qualifies now receives the full benefit. The specific income and resource thresholds for 2026 are published by Social Security, and they are higher than many people assume — certain things like your home, one car, and burial funds do not count as resources.

Because the limits change and because household size matters, the best way to know if you qualify is to apply and let Social Security do the math. There is no cost to apply, and being turned down once does not stop you from applying again later if your situation changes.

How does Extra Help work with the 2026 Part D rules?

Extra Help works alongside the broader Part D changes already in place. For 2026, the standard Part D deductible is capped at $615, and no enrollee pays more than $2,100 out of pocket for covered drugs in the year. The old coverage gap — the "donut hole" — was eliminated in 2025, leaving three simpler payment phases.

Extra Help goes further for those who qualify: instead of paying the standard deductible or the plan's regular copays, you pay small set amounts for generic and brand-name drugs, with those amounts set by Medicare. You can also still use the new Medicare Prescription Payment Plan to spread any remaining out-of-pocket drug costs across the calendar year in level monthly amounts, if that helps your budget.

How do you apply for Extra Help from Lancaster?

You can apply for Extra Help in four ways: online at ssa.gov, by calling Social Security at 1-800-772-1213, by mailing in a paper application, or in person at a Social Security office. There is no deadline — applications are accepted year-round, and approval is not tied to Medicare's Annual Enrollment Period of Oct. 15 through Dec. 7.

Once Social Security approves you, Medicare sends a letter explaining your copay level and when the help begins. You will still need to be enrolled in a Part D drug plan or a Medicare Advantage plan that includes drug coverage for the subsidy to apply at the pharmacy. Lancaster is in Dallas County, where a range of stand-alone Part D plans and Medicare Advantage plans with drug coverage are available for 2027; you choose the plan, and Extra Help attaches to it.

What should Lancaster residents do during the 2027 enrollment season?

If you think you might qualify, apply for Extra Help first — then compare Part D or Medicare Advantage drug plans during the Annual Enrollment Period, Oct. 15 to Dec. 7, for coverage that starts Jan. 1, 2027. If you are already in a Medicare Advantage plan, you also have the Medicare Advantage Open Enrollment Period from Jan. 1 through March 31 to switch to another Advantage plan or return to Original Medicare.

Keep an eye on your Annual Notice of Change letter, which arrives in late September. If your current drug plan's formulary or pharmacy network is changing for 2027, Extra Help makes it easier to switch, because people who get Extra Help have additional opportunities during the year to change Part D plans. One note on higher incomes: Part D's IRMAA surcharges begin above $109,000 in income for a single filer in 2026, which is well above the Extra Help limits — the two programs do not overlap.

Frequently asked questions

Do I have to be on Medicaid to get Extra Help?
No. People with both Medicare and full Medicaid are automatically enrolled in Extra Help, but you can also qualify on income and resources alone. Many people with Medicare only are eligible and do not realize it — the only way to know is to apply through Social Security.
Can I apply for Extra Help outside the Annual Enrollment Period?
Yes. Extra Help applications are accepted any time of year. You do not have to wait for Oct. 15 through Dec. 7. Once you are approved, you also get added chances to change your Part D plan during the year.
Will Extra Help pay my Medicare Advantage premium?
Extra Help lowers the drug (Part D) portion of your costs. If you are in a Medicare Advantage plan that includes drug coverage, Extra Help reduces the drug premium portion up to a benchmark amount and lowers your prescription copays, but it does not pay the medical portion of the premium or the Part B premium.
