- IRMAA is an extra amount added to Part B and Part D premiums for higher-income Medicare beneficiaries in 2027, not a penalty or mistake.
- Surcharges start above $109,000 in income for a single filer, or roughly double that for married couples filing jointly, and increase in steps for higher incomes.
- Social Security uses income from two years earlier to set your IRMAA bracket, so a one-time event like a home sale can affect your 2027 premiums.
- If your income has dropped due to a qualifying life event, you can file Form SSA-44 with Social Security to ask for a new IRMAA determination.

Baton Rouge retirees: here's how income changes what you pay for Medicare in 2027
BATON ROUGE — As Medicare's Annual Enrollment Period runs through Dec. 7, many Capital Region retirees are opening notices from Social Security that mention something called IRMAA — the Income-Related Monthly Adjustment Amount. It isn't a penalty, and it isn't a mistake. It's the extra amount higher-income beneficiaries pay for Medicare Part B and Part D in 2027.
If your income is modest, IRMAA won't touch you. But if you sold a rental in Mid City, cashed out a chunk of a 401(k), or took a large one-time distribution two years ago, it's worth understanding how the surcharge works before your 2027 premiums begin.
What IRMAA is, in plain English
Medicare Part B (doctor visits, outpatient care) and Part D (prescription drugs) have a standard monthly premium that most people pay. If your income is above a certain level, Medicare adds a surcharge on top of that standard premium. That surcharge is IRMAA.
Two important things to know:
- It's based on your tax return from two years ago. Your 2027 IRMAA is generally based on your 2025 IRS return (specifically, your Modified Adjusted Gross Income, or MAGI).
- It's tiered, not gradual. Cross a threshold by even one dollar and you move into the next bracket.
The 2027 income thresholds
For 2026, IRMAA surcharges begin above $109,000 in income for a single filer (roughly double that for a married couple filing jointly). From there, the surcharge steps up through several higher brackets, with the largest amounts applied to the highest incomes.
Social Security — not Medicare — sends the letter that tells you which bracket you fall into and what your 2027 Part B and Part D amounts will be. Both surcharges are billed on top of your regular premium, and the Part D piece is charged even if you get your drug coverage through a Medicare Advantage plan.
For the exact 2026 dollar figures at each bracket, check medicare.gov — the "Costs" page is updated as CMS finalizes numbers.
What this means for the Baton Rouge area
Baton Rouge is in East Baton Rouge Parish, where most Medicare beneficiaries pay only the standard Part B and Part D premiums — IRMAA affects a smaller slice of higher-income households. Louisiana's median retirement income is lower than the national average, so many parish residents will never see a surcharge.
Still, IRMAA tends to surprise people who had one unusual income year — a home sale in the Garden District, a lump-sum pension payout, a large Roth conversion, or a business sale. Because Medicare looks back two years, a 2025 event can shape your 2027 premiums.
If your income has dropped, you can ask for a review
This is the part many people miss. If you've had a life-changing event that reduced your income, you can file Form SSA-44 with the Social Security Administration to request a new IRMAA determination. Qualifying events include:
- Retirement or work stoppage
- Death of a spouse
- Divorce or annulment
- Loss of pension income
- Loss of income-producing property (not from your own choice, such as a disaster)
Given hurricane and flood history in South Louisiana, that last category is worth knowing. Bring documentation — a termination letter, a death certificate, a divorce decree — and the local Social Security office can process the appeal.
Other 2027 numbers worth knowing
Even if IRMAA doesn't apply to you, a few program-wide changes take effect in 2027:
- Part D out-of-pocket cap: $2,100 in 2026 and $2,400 in 2027. Once your out-of-pocket drug spending hits this amount, you pay nothing for covered drugs the rest of the year.
- Part D maximum deductible: $615 in 2026 and $700 in 2027.
- The coverage gap ("donut hole") is gone as of 2025 — Part D now has three phases instead of four.
- The Medicare Prescription Payment Plan lets you spread your drug costs into monthly installments across the year, at no extra charge. It's optional, and you have to opt in.

2026 IRMAA income brackets
If your income is above a threshold, you pay an income-related surcharge (IRMAA) on top of your Part B and Part D premiums. Your 2027 surcharge is based on your 2025 tax return. CMS publishes the 2027 brackets and surcharge amounts later in the fall.
| Income — single filer | Income — married, joint | Part B monthly premium | Part D surcharge |
|---|---|---|---|
| $109,000 or less | $218,000 or less | $202.90 | Plan premium only |
| $109,001 – $137,000 | $218,001 – $274,000 | $284.10 | + $14.50 |
| $137,001 – $171,000 | $274,001 – $342,000 | $405.80 | + $37.40 |
| $171,001 – $205,000 | $342,001 – $410,000 | $527.50 | + $60.40 |
| $205,001 – $500,000 | $410,001 – $750,000 | $649.30 | + $83.30 |
| Above $500,000 | Above $750,000 | $689.90 | + $91.00 |
