Medicare Part D
Extra Help Could Ease Marlborough Drug Costs in 2026
MARLBOROUGH, Mass. — For older adults juggling prescription bills, the biggest Medicare change heading into 2027 isn't a new plan or a shiny benefit. It's a federal program many people have never heard of — and even more never claim.

- Extra Help, also called the Low-Income Subsidy (LIS), is a federal program run by Social Security and Medicare that can lower what you pay for Part D prescription drugs in 2027.
- People with full Extra Help pay no more than a small set copay for each covered generic or brand-name drug instead of a percentage of the retail price.
- You automatically qualify for Extra Help if you already have certain other benefits, and if you're close to the income and resource limits, you're encouraged to apply anyway since you might still qualify.
- Extra Help works with any Part D plan you already have, there's no fee to apply, and once approved it usually continues each year as long as you still qualify.
It's called Extra Help, also known as the Low-Income Subsidy (LIS). And in 2027, it can meaningfully lower what people on Medicare pay for their Part D prescription drugs.
What Extra Help actually does
Extra Help is a federal program run through the Social Security Administration and Medicare. If you qualify, it can:
- Pay all or part of your Part D monthly premium (up to a benchmark amount).
- Lower or wipe out your Part D deductible.
- Cap what you pay at the pharmacy counter for covered drugs at just a few dollars per prescription.
- Remove any late-enrollment penalty you may have picked up for signing up for Part D late.
According to CMS, people with full Extra Help pay no more than a small set copay for each covered generic or brand-name drug. That's a very different experience from paying a percentage of a drug's retail price.
Why it matters heading into 2027
Two big Part D changes make Extra Help worth a fresh look as you consider 2027 coverage:
- A higher out-of-pocket cap. The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027. That cap is a real safety net — but it's still a stretch on a fixed income. Extra Help can bring most enrollees well below that number.
- A maximum Part D deductible. The Part D maximum deductible is $615 for 2026 and $700 for 2027. Extra Help can reduce or eliminate that deductible entirely for people who qualify.
- The old "donut hole" is gone. As of 2025, Part D has three simpler phases: deductible, initial coverage, and catastrophic. Extra Help interacts with those phases so qualifying enrollees pay very little at each step.
You may also hear about the Medicare Prescription Payment Plan, a separate option that lets anyone with Part D spread their drug costs into monthly installments instead of paying a big bill at the pharmacy. It doesn't lower your total cost — Extra Help does that — but it can smooth out the timing.
Who qualifies in Massachusetts
Extra Help has income and resource limits set each year by the federal government. As a general guide for 2027, you may qualify if:
- Your income is at or below roughly 150% of the federal poverty level, and
- Your savings and resources are under the federal limits (retirement accounts, one home, one car, and burial funds generally don't count).
You automatically get Extra Help if you have Medicare and any of the following:
- Full Medicaid (MassHealth for Massachusetts residents),
- A Medicare Savings Program through your state, or
- Supplemental Security Income (SSI).
If you're close to the limits, apply anyway. Many people who assume they earn "too much" are surprised to find they qualify.
How Marlborough fits in
Marlborough is in Middlesex County, where Medicare beneficiaries choose among a range of stand-alone Part D drug plans and Medicare Advantage plans that include drug coverage. Plan availability is set at the county level by CMS, not city by city — so a Marlborough resident and a neighbor a few towns over generally see the same menu. The 2027 lineup is detailed in each plan's Annual Notice of Change and on Medicare.gov's Plan Finder.
That matters because Extra Help works with whichever Part D plan you're enrolled in. You don't have to switch to a special plan to use it, though during the Annual Enrollment Period (Oct. 15 – Dec. 7, 2026) it's smart to check that your current plan still covers your medications well for 2027.
How to apply
There's no fee to apply, and you can do it in whichever way is easiest:
- Online: ssa.gov/extrahelp
- By phone: Social Security at 1-800-772-1213
- In person: at your local Social Security office
- With free help: through SHINE, Massachusetts' State Health Insurance Assistance Program (SHIP), which offers unbiased Medicare counseling at no cost
If you're denied, you can reapply anytime your income or resources change.
A quick FAQ
Do I have to reapply every year? Not usually. Once you're approved, Extra Help continues as long as you still qualify. Social Security may send a review form; return it on time to keep your benefit.
I have MassHealth. Do I still need to apply? No. Full MassHealth automatically enrolls you in full Extra Help. But it's worth confirming with 1-800-MEDICARE that the subsidy is showing on your Medicare record.
Will Extra Help pay for drugs my plan doesn't cover? No. Extra Help only lowers costs for drugs that are on your Part D plan's formulary. If a medication isn't covered, ask your prescriber about an alternative or a formulary exception.
Does Extra Help affect my Medicare Advantage plan? It doesn't change your medical benefits. It applies to the Part D (drug) portion of any plan that includes drug coverage.
