What is Medigap and how is it different from Medicare Advantage in Illinois?

Medigap, also called Medicare Supplement insurance, is a private policy that works alongside Original Medicare (Part A and Part B) to help pay cost-sharing like coinsurance and deductibles. It is not the same as Medicare Advantage, which replaces the way Original Medicare pays claims by enrolling you in a private plan that bundles Part A, Part B and usually Part D.

With Medigap, you keep Original Medicare and can see any provider in the country who accepts Medicare. Medigap does not include a drug benefit, does not use provider networks the way Advantage plans do, and does not require referrals. The trade-off is that Medigap generally has a monthly premium in addition to your Part B premium, while most Advantage plans fold additional benefits into a single plan.

Which Medigap insurers and plan letters are listed for Illinois in 2027?

Medicare.gov's Medigap policy search lists 29 insurers for ZIP code 60601, with plan letters A, B, C, D, F, F-HD, G, G-HD, K, L, M and N appearing across the market. Plans C and F are closed to people newly eligible for Medicare on or after January 1, 2020.

The insurers listed, in alphabetical order, with the plan letters shown for each: AARP – UnitedHealthcare Insurance Company (A, B, C, F, G, K, L, N); AARP – UnitedHealthcare Insurance Company of America (A, F, G, N); Aetna Health Insurance Company (A, B, F, G, G-HD, N); AFLAC (A, F, G, N); American Benefit Life Insurance Company (A, F, G, N); Atlantic Capital Life Assurance Company (A, F, G, G-HD, K, N); Bankers Life, underwritten by Washington National Insurance Company (A, F, G, G-HD, N); BlueCross BlueShield of Illinois (A, F, F-HD, G, G-HD, N); Erie Family Life Insurance Company (A, F, G, N); Everence Association Inc. When is the Medigap Open Enrollment Period in Illinois and why does it matter?

The Medigap Open Enrollment Period is a one-time, six-month window that starts the month you are both 65 or older and enrolled in Medicare Part B. During it, you can buy any Medigap policy sold in Illinois from any insurer listed, without medical underwriting.

Outside that window, insurers in most situations can look at your health, charge more, or decline to issue a policy. That is why the six-month window matters so much: inside it, your health history generally cannot be used against you.

What happens to Medigap rights if my Medicare Advantage plan leaves Illinois for 2027?

According to CMS Landscape files, 44 Medicare Advantage plans that are offered in Illinois in 2026 will not be offered anywhere in the state in 2027. If you are enrolled in a plan that is not returning, you have a federal guaranteed-issue right to buy certain Medigap policies, which means an insurer must sell you one of the eligible plans and cannot use your health to decide the price.

Other guaranteed-issue situations exist too — for example, losing employer retiree coverage. Medicare.gov describes each one in detail.

Does Medigap cover prescriptions in Illinois, or do I need a separate Part D plan?

Medigap policies sold today do not cover prescription drugs, so Illinois residents who want drug coverage with Original Medicare and a Medigap plan sign up for a stand-alone Part D plan separately. For 2027, the Part D out-of-pocket cap is $2,400 (up from $2,100 in 2026), and the most any Part D plan may charge as a deductible is $700. The Part D base beneficiary premium used in the late-enrollment penalty calculation is $41.33; actual plan premiums are set plan by plan.

Part D choices for 2027 are made during Annual Enrollment 2027, which runs October 15 – December 7, 2026, with coverage effective January 1, 2027.

How should I compare Medigap policies in Illinois?

Start with the standardized plan letter, because benefits for a given letter are the same from any insurer in Illinois; what differs is price, customer service and how the insurer sets future rate increases. From there, compare quotes from several insurers for the same letter, confirm the policy is available in your ZIP code, and ask how the insurer prices by age.

The Medicare Plan Finder at medicare.gov lets you pull up Medigap options by ZIP code, and plan or insurer documents will show the full terms. The Social Security Administration announced a 3.6 percent cost-of-living adjustment for benefits payable beginning January 2027, which may affect your household budget as you compare.

Where can Illinois residents get free help with Medigap?

Free, personalized help is available from Medicare itself. You can call 1-800-MEDICARE (1-800-633-4227), 24 hours a day, seven days a week, or use the tools at medicare.gov to compare Medigap policies, Part D plans and Medicare Advantage plans side by side.
