Anthem's 2027 Medicare Advantage lineup in Glendale: 5 plans, mostly for special needs
GLENDALE, Ariz. — As the Annual Enrollment Period for 2027 opens on Oct. 15, 2026, Anthem is bringing five Medicare Advantage plans to Maricopa County, the county that includes Glendale. Four of the five are Special Needs Plans, meaning they are built for people who meet specific health or living-situation rules — not for the general Medicare population. One plan is new for next year, and one plan Anthem offered this year will not return in 2027.

If you are already in an Anthem plan here, your Annual Notice of Change (ANOC) is the document that spells out what is different about your plan for 2027. If you are shopping, the table below lists every Anthem plan available in the county next year, and this article walks through what each type means and how to check a plan against your own doctors and prescriptions before Dec. 7, 2026.

What Medicare Advantage plans does Anthem offer in Glendale for 2027?
Anthem offers 5 Medicare Advantage plans in Maricopa County, the county Glendale sits in, for the 2027 plan year. By type, that is 2 HMO plans and 3 HMO-POS plans. All 5 include Part D prescription drug coverage.
CMS groups Anthem under its parent, Elevance Health, in the public Landscape files. The full plan names, exactly as they appear on Medicare.gov, are shown in the table on this page. Anthem is one of 16 carriers offering Medicare Advantage in Maricopa County next year; across all carriers there are 92 plans in the county for 2027.
Which Anthem plans are Special Needs Plans in Maricopa County?
Four of Anthem's five Maricopa County plans are Special Needs Plans (SNPs): two are Chronic Condition SNPs (C-SNPs) and two are Institutional SNPs (I-SNPs). Anthem does not offer a Dual Eligible SNP (D-SNP) in the county for 2027.
A C-SNP is limited to people living with a qualifying chronic condition the plan is designed for. An I-SNP is limited to people who live in (or need the level of care provided in) a long-term care setting such as a nursing facility, or who qualify for nursing-home level care at home. If you do not meet the eligibility rule for a SNP, you cannot enroll in it, even during AEP. The one non-SNP plan in Anthem's local lineup is a standard HMO-POS open to anyone with Medicare Parts A and B who lives in the service area.
What is new or not returning in Anthem's 2027 Glendale lineup?
For 2027, Anthem has 1 new plan and 1 plan that is not returning in Maricopa County. If the plan you have today is the one being discontinued, Anthem is required to send you a non-renewal notice explaining your options and the Special Enrollment Period you qualify for.
For every renewing plan, the ANOC — mailed by late September — lists what changes on Jan. 1, 2027: premium, deductible, drug tiers, provider network, extra benefits. Reading your ANOC is the single most useful 20 minutes you can spend this fall. If you cannot find yours, call the number on your member ID card and ask for it.
How do I check an Anthem plan against my doctors and prescriptions before Dec. 7?
The fastest way is the Plan Finder at Medicare.gov. Enter your ZIP code, add your prescriptions and preferred pharmacy, and the tool will show each plan's estimated yearly drug cost, formulary tier for every drug on your list, and whether your pharmacy is in the preferred network.
A few steps that save headaches later:
- Have your Medicare number and a current list of your medications (name, dose, how often) in front of you.
- For doctors and hospitals, do not rely on the Plan Finder alone — open the plan's own provider directory and call the office to confirm they will accept the specific plan for 2027.
- For drugs, look up each one on the plan's 2027 formulary (not the 2026 one) because tiers and prior-authorization rules can change year to year.
- If something is unclear, call 1-800-MEDICARE (TTY 1-877-486-2048), available 24/7.
If you do nothing by Dec. 7, 2026, and your current plan is being offered again next year, you will be auto-renewed into it with the changes described in your ANOC.
What else should I know about costs and deadlines for 2027?
Two program-wide 2027 figures are already set by CMS: the Part D out-of-pocket cap rises to $2,400 (up from $2,100 in 2026), and the Part D maximum deductible rises to $700 (up from $615 in 2026). The Part D national base beneficiary premium for 2027 is $41.33 — that is a formula input used across the program, not a price any specific plan charges.
The 2027 standard Part B premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes those in the fall, usually November. Until then, do not rely on any number you see projected elsewhere.
The dates to remember: Annual Enrollment runs Oct. 15 – Dec. 7, 2026, for coverage starting Jan. 1, 2027. If you end up in a Medicare Advantage plan and change your mind, the Medicare Advantage Open Enrollment Period runs Jan. 1 – March 31, 2027, and lets you switch to another MA plan or back to Original Medicare once.
