A Special Needs Plan (SNP) is a Medicare Advantage plan limited to people who meet specific eligibility. A C-SNP, like Humana's two diabetes-and-heart plans in Maricopa County, is for people living with certain chronic conditions. If you take no action by Dec. 7, 2026, and your current plan is being offered again in 2027, you stay in it automatically — with whatever changes to premium, benefits, network or drug list are described in your ANOC. Since none of Humana's current Maricopa County plans are leaving for 2027, enrollees in those plans will not be forced to switch.

If you later decide the plan is not the right fit, Medicare Advantage members have a second window: the Medicare Advantage Open Enrollment Period, Jan. 1 through March 31, 2027, when you can switch to another Medicare Advantage plan or return to Original Medicare once.
