Anaheim, Calif. — Fall 2026. If you have both Medicare and Medi-Cal and live in Anaheim, your choices for 2027 Dual Special Needs Plans (D-SNPs) are set. In Orange County, three D-SNPs will be offered next year, from three different carriers. The Annual Enrollment Period runs October 15 through December 7, 2026, and any change you make takes effect January 1, 2027 — but as a dual-eligible person, you also get extra chances to switch during the year.
What is a D-SNP, and who qualifies in Anaheim?
A Dual Special Needs Plan is a type of Medicare Advantage plan built for people who have both Medicare and Medicaid (called Medi-Cal in California). It coordinates your Medicare benefits with the help you get from Medi-Cal.
You can be a "full dual" (Medi-Cal pays for a wide range of services, including long-term care) or a "partial dual" (Medi-Cal helps pay your Medicare premiums or cost-sharing through a Medicare Savings Program). Different D-SNPs are designed for different eligibility levels, so the plan has to match the kind of Medi-Cal help you receive.
Your Medi-Cal status is confirmed by the state — in California, that is the Department of Health Care Services, often through your county Medi-Cal office. Your Medicare status is confirmed by the Social Security Administration and CMS. - Centene (WellCare) — Wellcare Health Net Dual Liberty (HMO D-SNP) — renamed for 2027 (previously Wellcare Dual Liberty)

How often can a dual-eligible person change plans?
Most people with Medicare can only change Medicare Advantage or Part D plans during the Annual Enrollment Period (Oct. 15–Dec. 7) or the Medicare Advantage Open Enrollment Period (Jan. 1–March 31). Dual-eligible people have more flexibility.
If you have Medicaid (Medi-Cal) or Extra Help with Part D, you qualify for a Special Enrollment Period that lets you make a change once per calendar quarter during the first three quarters of the year. On top of that, there is a monthly opportunity to either join an integrated D-SNP that aligns with your Medi-Cal plan, or move to Original Medicare with a stand-alone Part D plan.
These rules have changed in recent years and can change again. Before you switch, confirm the current Special Enrollment Period rules by calling 1-800-MEDICARE (TTY 1-877-486-2048), 24 hours a day, 7 days a week, or by checking medicare.gov.
How do I confirm I'm eligible for a D-SNP in California?
Start with two phone calls:
1. 1-800-MEDICARE — to confirm your Medicare status and current enrollment, and to see which D-SNPs you qualify for based on your Medi-Cal level. 2. Your county Medi-Cal office or the California Department of Health Care Services — to confirm your Medi-Cal category (full-scope, QMB, SLMB, QI, or another level). This is what determines which D-SNPs you can join.
You can also compare plans side by side at medicare.gov using the Plan Finder; it will ask whether you have Medicaid and Extra Help, and it will show the D-SNPs available in your ZIP code.
What else is changing for 2027?
A few program-wide numbers are already confirmed for 2027:
- The Part D out-of-pocket cap rises to $2,400 in 2027 (it is $2,100 in 2026).
- The Part D maximum deductible rises to $700 in 2027 (it is $615 in 2026).
- The Part D national base beneficiary premium is $41.33 for 2027 — this is a formula input, not what any plan actually charges.
The 2027 Part B standard premium and deductible, along with the IRMAA income brackets, have not been announced yet. CMS usually publishes them in the fall, often in November. We will not estimate them here.
If you do nothing during AEP, your current plan will renew for 2027 if it is still offered — with any changes spelled out in the Annual Notice of Change (ANOC) your plan mailed you in September. Read that ANOC before you decide to stay or switch.
