Irvine is in Orange County, where 111 Medicare Advantage plans from 16 carriers are offered for 2027. By plan type, that breaks down to 84 HMOs, 23 HMO-POS plans and 4 PPOs. The type you pick shapes which doctors you can see, whether you need a referral, and what happens if you need care outside the plan's network.

What do HMO, HMO-POS, PPO and PFFS mean for Medicare Advantage?

Each label is a set of rules about networks and referrals. An HMO (Health Maintenance Organization) uses a defined local network and generally asks you to pick a primary care doctor; care from outside the network is typically not covered except for emergencies. An HMO-POS (Point of Service) is an HMO with a limited "point of service" option that lets you go outside the network for certain services, usually at a higher cost. Humana is the only carrier offering PPO plans in Orange County for 2027, according to the CMS Landscape file. Those four PPOs are the entirety of the county's PPO shelf next year.

A PPO is often the choice for people who split time between states, travel often, or want to use providers outside a single network without a referral. Out-of-network care is covered, but at a higher cost than in-network care. If broad flexibility matters to you, it is worth checking whether the doctors and hospitals you actually use are in a given PPO's network — PPOs still have networks, they just do not lock you into them.
