What Kaiser Permanente is offering in Sacramento County for 2027
Sacramento is in Sacramento County, where Medicare Advantage plan availability is set. For plan year 2027, Kaiser Permanente's lineup here looks like this:
- Kaiser Permanente Dual Complete North P2 (HMO D-SNP) — HMO D-SNP, with drug coverage
- Kaiser Permanente Sr Adv Enhanced Sac., Sonoma (HMO) — HMO, with drug coverage
- Kaiser Permanente Sr Advantage Basic Sac., Sonoma (HMO) — HMO, with drug coverage
The table below lists every plan in the lineup. All three are HMOs — meaning you generally use the plan's network of doctors and hospitals and pick a primary care physician — and all three include Part D prescription drug coverage built in.
The Special Needs Plan in the mix
One of the three plans, Kaiser Permanente Dual Complete North P2, is a Dual Eligible Special Needs Plan, or D-SNP. D-SNPs are built for people who have both Medicare and full Medi-Cal (California's Medicaid) and have their own eligibility rules. If you are not dually eligible, this plan is not for you; if you are, it is one option to look at alongside others in the county.
Kaiser Permanente is not offering a Chronic Condition SNP (C-SNP) or an Institutional SNP (I-SNP) in Sacramento County for 2027.
What is new, renamed or not returning for 2027
For Sacramento County, there are no changes to Kaiser Permanente's plan list year over year: zero plans not returning, zero new plans, zero renamed. The names and plan types match 2026.
That does not mean nothing changes. Every Medicare Advantage plan can adjust its premium, deductible, copays, drug list (formulary), provider network and extra benefits from one year to the next. Those details arrive in your Annual Notice of Change (ANOC), which plans mail by late September. If you have not read yours, that is the single most useful piece of paper on your table this fall.
How Kaiser Permanente fits into the Sacramento County market
Kaiser Permanente is one of 10 carriers offering Medicare Advantage plans in Sacramento County for 2027. All together, those carriers offer 32 plans in the county. So Kaiser Permanente's three plans sit inside a wider local market, and comparing across carriers — not just within one — is how most people land on the right fit.
How to check a plan against your doctors and prescriptions before December 7
Before you choose or stay, run the plan through three quick checks:
1. Doctors and hospitals. Confirm your primary care doctor, specialists and preferred hospital are in-network for 2027. HMO plans generally require in-network care except for emergencies. Check the plan's online provider directory or call the plan. 2. Prescriptions. Put every medication you take into the Medicare Plan Finder at medicare.gov. The tool shows the 2027 formulary, tier, and your estimated yearly drug cost under each plan. 3. Total cost, not just premium. Look at the premium, the deductible, your doctor and drug copays, and the plan's maximum out-of-pocket limit together. A lower premium can mean higher costs when you use care, and vice versa.
A few 2027 program-wide numbers worth knowing as you compare drug coverage: the Part D out-of-pocket cap rises to $2,400 in 2027 (up from $2,100 in 2026), the Part D maximum deductible is $700 (up from $615), and the Part D national base beneficiary premium — a formula input, not what any plan charges — is $41.33. CMS has not yet announced the 2027 Part B standard premium and deductible or the 2027 IRMAA brackets; those typically come out later in the fall.
What happens if you do nothing
Nothing dramatic. If your current Kaiser Permanente plan is on the 2027 list — and all three are — it renews on January 1, 2027 with the changes described in your ANOC. If you want to switch to a different Medicare Advantage plan, change carriers, or move to Original Medicare with a stand-alone Part D plan, you have until December 7, 2026. After that, Medicare Advantage members get one more window — the Medicare Advantage Open Enrollment Period, January 1 through March 31, 2027 — to make a single switch.
