MyMedicarePlans.org is an independent consumer site; we are not Aetna and not Medicare. Aetna is one of 12 carriers offering Medicare Advantage plans in San Francisco County for 2027 (46 plans in all). This piece looks only at Aetna's slice of that list.
What Aetna is offering in San Francisco County for 2027
The table on this page lists every Aetna plan available in the county for next year. The short version: 4 Medicare Advantage plans, broken down as 3 HMOs and 1 HMO-POS. All 4 include Part D prescription drug coverage. One of them, Aetna Medicare Chronic Care (HMO C-SNP), is a Chronic Condition Special Needs Plan — a plan designed for people living with certain chronic conditions. Aetna is not offering a Dual Eligible SNP (D-SNP) or an Institutional SNP (I-SNP) in the county for 2027.
For the plan-type labels: an HMO generally asks you to use a network of doctors and hospitals and to pick a primary care doctor. An HMO-POS (Point of Service) is an HMO that lets you go out of network for certain services, usually at a higher cost. The plan's Evidence of Coverage spells out exactly how each one works.
What's new, what's not returning, and what's renamed
Three Aetna plans offered in San Francisco County in 2026 are not being offered for 2027. If you are enrolled in one of them, Aetna is required to send you a non-renewal notice this fall explaining your options and your Special Enrollment Period rights. You will have time to pick another plan without a gap in coverage, but it is on your calendar now, not later.
Two Aetna plans are new for 2027: Aetna Medicare Chronic Care (HMO C-SNP) and Aetna Medicare Eagle (HMO). "New" means the plan did not exist in the county last year, so there is no prior version to compare it to — you'll want to read the plan documents fresh.
One plan has been renamed: Aetna Medicare Signature Extra, which was offered as an HMO-POS in 2026, is being offered under the same name as an HMO-POS for 2027. A rename or plan-type relabel on paper can still come with real changes to the network, drug list or cost sharing, so treat it like a new plan when you review it.
Two plan names carry over from 2026 to 2027: Aetna Medicare Signature (HMO) and Aetna Medicare Signature Extra. Your ANOC is the document that shows, side by side, what is changing in your specific plan between this year and next.
How to check a plan against your doctors and prescriptions
Before December 7, two checks matter more than any brochure:
1. Your doctors and hospitals. Each plan has its own provider network. The Medicare Plan Finder at medicare.gov lets you enter a plan and search for a provider, but the most reliable source is the plan's own online directory — and a quick call to your doctor's billing office to confirm they will accept the specific plan for 2027.
2. Your prescriptions. Each plan has its own formulary (drug list) and its own preferred pharmacies. On medicare.gov, you can enter your drugs and your pharmacy and see how each plan would cover them. Formularies can change year to year, so a drug that was covered on your 2026 plan is not guaranteed to be covered the same way in 2027.
If your plan is one of the three not returning, those same two checks apply to any replacement plan you are considering.
The 2027 program-level numbers worth knowing
A few Medicare figures for 2027 are already set by CMS and apply across every Part D plan, including the drug coverage inside Medicare Advantage plans:
- The Part D out-of-pocket cap on covered drugs is $2,400 in 2027 (up from $2,100 in 2026).
- The Part D maximum deductible a plan can charge is $700 in 2027 (up from $615 in 2026).
- The Part D national base beneficiary premium is $41.33 for 2027 — this is a formula input Medicare uses, not what any particular plan will charge you.
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS usually publishes those figures later in the fall.
