What's new, what's renamed, what's not returning

Two Blue Shield plans are new in Los Angeles County for 2027: Blue Shield Advantage (HMO) and Blue Shield Value Advantage Plan (HMO). Both are HMOs with drug coverage.

One plan has been renamed: the plan now called Blue Shield Medicare + Medi-Cal Plan (HMO D-SNP) was known in 2026 as Blue Shield TotalDual Plan (HMO D-SNP). The name is different; the Annual Notice of Change explains what else, if anything, is different about the plan.

No Blue Shield plans are leaving Los Angeles County for 2027. If you're on a Blue Shield plan today and it is not on the 2027 list under the same name, check the ANOC Blue Shield mailed you in September — a rename, not a cancellation, may be the reason.

How to check a plan against your doctors and prescriptions

Before December 7, it's worth spending 20 minutes to confirm a plan actually works for you — not in the brochure sense, but in the practical sense. Three things matter most:

1. Your doctors. A plan's provider network can change year to year. Confirm each of your doctors and your preferred hospital are in-network for the specific plan you're considering, for 2027, not 2026. The plan's own provider directory is the source; a call to your doctor's billing office is a good second check.

2. Your prescriptions. Each plan has its own formulary (drug list) and its own tier placement. Enter your medications into the Medicare Plan Finder at medicare.gov to see how each plan would cover them in 2027, including any step therapy or prior authorization.

3. Your pharmacy. Plans have preferred and standard pharmacies, and the cost can differ. Make sure the pharmacy you use is on the plan's preferred list, or be ready to switch.

Program-level numbers to keep in mind

A few 2027 figures from CMS are already published and apply to Part D drug coverage regardless of which plan you pick:

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes them later in the fall.

The coverage gap (the old "donut hole") is gone — Part D has had three phases since 2025. And the Medicare Prescription Payment Plan lets anyone with Part D spread drug costs across the calendar year in monthly amounts if they want to.

If you do nothing

Nothing happens to you if you don't act. Your current plan renews on January 1, 2027, as long as it's offered next year, with the changes described in the Annual Notice of Change your plan sent you. Since none of Blue Shield's 2026 plans are leaving Los Angeles County, current Blue Shield members in Long Beach will have their plan continue — under its current name, or under the new name in the case of the D-SNP.
