What Champion Heath Plan is offering in Kern County for 2027

The one plan on the list that is not a Special Needs Plan is Champion Ally (HMO), which is a general-enrollment Medicare Advantage plan. Champion Heath Plan is not offering a Dual Eligible SNP (D-SNP) or an Institutional SNP (I-SNP) in Kern County for 2027.

Your prescriptions. Enter your drugs into the Plan Finder at medicare.gov. It will show you each plan's formulary, the tier your drug falls on, and the estimated yearly cost. If a drug is not covered, ask whether a similar drug on the formulary would work. 3. Your eligibility, if it is a C-SNP.
