What's new, what's renamed, what's leaving

Two Aetna plans are new to the San Diego County lineup for 2027: the Aetna Medicare Chronic Care (HMO C-SNP) and the Aetna Medicare Eagle (HMO). The Chronic Care plan is a C-SNP — a Chronic Condition Special Needs Plan — which is designed for people with specific qualifying health conditions. Aetna is not offering a D-SNP (for people with both Medicare and Medicaid) or an I-SNP (for people living in an institution) in San Diego County for 2027.

Two returning plans have been renamed. Aetna Medicare Prime (HMO) is the new name for what was Aetna Medicare Prime (HMO-POS), and Aetna Medicare Signature Legacy (HMO) is the new name for what was Aetna Medicare Signature (HMO). If you are currently in either plan, read your Annual Notice of Change carefully — a name change can come with other changes too.

Five Aetna plans sold in San Diego County in 2026 are not returning for 2027. If you are enrolled in one of them, Aetna will send you a notice explaining your options. You will not be left without coverage automatically, but you will need to pick a new plan during the Annual Enrollment Period, or you will be moved back to Original Medicare.

How to check a plan against your doctors and prescriptions

Before December 7, it is worth taking 20 minutes to do three things:

1. Pull out your Annual Notice of Change (ANOC). It arrived in September. It lists every change — premium, deductible, copays, drug tiers, network — your current plan is making for 2027. 2. Check your doctors. Call each provider's billing office and ask whether they will be in-network for the specific plan name you are considering in 2027. Networks change on January 1, and a provider directory from today may not reflect next year. 3. Check your prescriptions. Use the Medicare Plan Finder at medicare.gov to enter the drugs you take and the pharmacy you use. Plan Finder shows you the 2027 formulary and the estimated yearly cost for each plan.

The other numbers to know for 2027

CMS has confirmed a few program-level figures for 2027 that apply no matter which plan you pick:

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes those in the fall, usually in November. We will not estimate them here.

The donut hole has been gone since 2025, replaced by a three-phase Part D benefit. If large drug bills hit early in the year, the Medicare Prescription Payment Plan lets you spread what you owe across the calendar year in monthly installments at no extra cost.

You have options beyond one carrier

What happens if my Aetna plan is one of the five not returning in 2027?
Aetna will send you a non-renewal notice this fall explaining that your plan will end on December 31, 2026. You can use the Annual Enrollment Period (October 15 – December 7, 2026) to pick a different Medicare Advantage plan, join a stand-alone Part D plan with Original Medicare, or make another change. If you do nothing, you will be returned to Original Medicare on January 1, 2027, without drug coverage.
My plan was renamed. Do I need to re-enroll?
No. A rename alone does not require you to do anything. But a new name often comes with other changes to premiums, copays, drug lists or the provider network, and those are listed in your Annual Notice of Change. Read it before December 7 so you can switch if the plan no longer fits.
What is a C-SNP, and can anyone join one?
A Chronic Condition Special Needs Plan is a type of Medicare Advantage plan built for people with a specific qualifying chronic condition, such as diabetes or heart failure. You generally need a doctor to verify the condition. If you do not qualify, you cannot enroll.
When do I have to decide by?
December 7, 2026. Changes made during the Annual Enrollment Period take effect January 1, 2027.
