Understanding the plan types
An HMO (Health Maintenance Organization) typically asks you to pick a primary care doctor and use the plan's network, except in emergencies. Referrals are often required to see specialists.
A D-SNP (Dual-eligible Special Needs Plan) is a type of Medicare Advantage plan built for people who qualify for both Medicare and Medicaid (Medi-Cal in California). Enrollment is limited to people who meet both programs' eligibility rules.

- Part D national base beneficiary premium: $41.33 for 2027 (this is a formula input, not what any specific plan charges)
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes them later in the fall, usually in November. We'll update coverage when they're released.
The coverage gap (the old "donut hole") was eliminated in 2025; Part D now has three phases. If drug costs are a concern, ask about the Medicare Prescription Payment Plan, an optional program that lets you spread your out-of-pocket drug costs across the calendar year in monthly installments rather than paying at the pharmacy counter.
