What Alignment Health Plan is offering in San Francisco for 2027
San Francisco is in San Francisco County, where Medicare Advantage availability is set. For 2027, Alignment Health Plan has 12 plans in the county:

What is new, what is leaving, and what has been renamed
Three things changed in Alignment's local lineup between 2026 and 2027:
- New for 2027: Alignment Health Honor Plus 060 (HMO).
- Not returning for 2027: one 2026 Alignment plan is no longer being offered. If you are enrolled in a plan that is leaving, Medicare gives you a Special Enrollment Period to pick a new plan — but you still want to act before coverage ends.
- Renamed: most of Alignment's returning plans have new names for 2027. For example, Alignment Health Harmony (HMO) is now listed as Central Health Harmony 031 (HMO), Alignment Health CalPlusDuals (HMO D-SNP) is now Alignment Health Duals+ 030 (HMO D-SNP), and the Heart & Diabetes family has been reorganized into several numbered plans. A rename does not automatically mean a benefit change — but it does not rule one out either. Your ANOC is the document that tells you what actually changed.
The 2027 program numbers that apply to every Part D plan
Alignment's plans all include drug coverage, so the Part D rules Medicare sets each year will shape what you pay at the pharmacy. For 2027, CMS has confirmed:
- Part D out-of-pocket cap: $2,400 (up from $2,100 in 2026). Once you hit that cap, you pay $0 for covered drugs the rest of the year.
- Part D maximum deductible: $700 (up from $615 in 2026). Plans can set a lower deductible.
- The Medicare Prescription Payment Plan is still available — it lets you spread drug costs across the year in monthly bills instead of paying all at once at the pharmacy.
The 2027 Part B premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS usually releases those in November. We are not estimating them.
How to check an Alignment plan against your doctors and prescriptions
Before December 7, 2026, it is worth running three quick checks on any plan you are considering — new or current:
1. Doctors and hospitals. HMO and HMO-POS plans use networks. Call each provider's billing office and ask whether they are contracted with the specific Alignment plan for 2027. "In network with Alignment" is not enough; plans and networks can differ. 2. Prescriptions. Each plan has its own formulary (drug list) and tier structure. Use the Medicare Plan Finder at medicare.gov — enter your drugs and pharmacy to see how each plan would cover them in 2027. 3. Your situation. The Special Needs Plans in the list have eligibility rules — a D-SNP requires Medicare and Medi-Cal; a C-SNP requires a qualifying chronic condition. You cannot enroll in one unless you qualify.
If you do nothing, your current Alignment plan renews for 2027 as described in the ANOC it mailed you. Nothing lapses automatically.
