Wellcare's 2027 Medicare Advantage plans in San Diego: the full lineup, with two renamed
San Diego, Calif. — As the fall 2026 Annual Enrollment Period runs through December 7, Wellcare is offering four Medicare Advantage plans to people on Medicare in San Diego County for 2027. The lineup is the same size as this year, with no plans leaving and no new ones added — but two of the four carry new names, which means the Annual Notice of Change in the mail is worth a careful read.

What Wellcare Medicare Advantage plans are available in San Diego County for 2027?
Four plans, from one carrier. In San Diego County, Centene — which CMS lists as Centene (WellCare) — is offering four Medicare Advantage plans for 2027. All four are HMOs, and all four include Part D prescription drug coverage.
The table on this page lists each one. In alphabetical order, they are:
- Wellcare Health Net Dual Liberty (HMO D-SNP) — HMO D-SNP, with drug coverage
- Wellcare Low Premium (HMO) — HMO, with drug coverage
- Wellcare Low Premium Focus (HMO) — HMO, with drug coverage
- Wellcare Specialty Simple (HMO C-SNP) — HMO C-SNP, with drug coverage
San Diego is in San Diego County, and Medicare Advantage availability is set at the county level, so these are the Wellcare options for anyone enrolling from a San Diego ZIP code.
What is new or not returning from Wellcare in San Diego for 2027?
Nothing is leaving and nothing is brand new, but two plan names have changed. For 2027, no Wellcare plans in San Diego County are discontinued, and no plans are new to the market. Two are renamed:
- Wellcare Dual Liberty (HMO D-SNP) is now Wellcare Health Net Dual Liberty (HMO D-SNP).
- The 2026 plan called Wellcare Low Premium (HMO) now appears as Wellcare Low Premium Focus (HMO). (A different plan now carries the "Wellcare Low Premium (HMO)" name for 2027.)
A rename does not automatically mean the plan is the same inside. The Annual Notice of Change (ANOC) your plan mailed in September is the official document that spells out what shifted for 2027 — premium, drug list, copays, provider network, extra benefits. If you cannot find your ANOC, call the number on your member card and ask for one.
What are Special Needs Plans, and which ones does Wellcare offer here?
Special Needs Plans are Medicare Advantage plans built for a specific group of people. In San Diego County for 2027, Wellcare offers two:
- A D-SNP (Dual Eligible Special Needs Plan) for people who have both Medicare and full Medi-Cal — that is Wellcare Health Net Dual Liberty (HMO D-SNP).
- A C-SNP (Chronic Condition Special Needs Plan) for people living with a qualifying chronic condition — that is Wellcare Specialty Simple (HMO C-SNP).
You can only enroll in an SNP if you meet the eligibility rules for that specific group. Medicare.gov's Plan Finder will tell you whether you qualify before you apply, and so will 1-800-MEDICARE.
How do I check a plan against my doctors and prescriptions before December 7?
Use Medicare's Plan Finder at medicare.gov/plan-compare — it is free, official, and built for exactly this. Three steps make the comparison honest:
1. Enter your drugs and pharmacy. Type each prescription, dose and quantity. The Plan Finder will show the expected yearly drug cost under each plan, including deductible and coinsurance phases. 2. Check your doctors. Plan Finder links out to each plan's provider directory, but the surest way is to call your doctor's office and ask, by plan name, whether they will be in network on January 1, 2027. Networks change at the plan year. 3. Read the Summary of Benefits. For any plan you are seriously considering, open its Summary of Benefits (linked from Plan Finder) and its Evidence of Coverage for the specifics on copays, referrals and prior authorization.
If you prefer to talk to a person, 1-800-MEDICARE (TTY 1-877-486-2048) is open 24/7.
What else should San Diego residents know for the 2027 plan year?
A few program-wide facts that will matter when you compare plans:
- The Annual Enrollment Period runs October 15 to December 7, 2026. Any change you make takes effect January 1, 2027.
- If you do nothing, your current plan renews — with whatever changes are in its ANOC.
- The Part D out-of-pocket cap rises to $2,400 in 2027 (up from $2,100 in 2026). Once your out-of-pocket drug spending hits that cap, you pay $0 for covered Part D drugs the rest of the calendar year.
- The Part D maximum deductible is $700 in 2027 (up from $615 in 2026). Plans can charge less; they cannot charge more.
- The Medicare Prescription Payment Plan is still available — it lets you spread Part D out-of-pocket costs across the calendar year in monthly installments instead of paying at the pharmacy counter.
- The 2027 Part B premium and deductible and the 2027 IRMAA income brackets have not been announced yet; CMS typically publishes them in November.
- Wellcare is one of 15 carriers offering Medicare Advantage in San Diego County for 2027, which together list 85 plans. If a Wellcare plan is not the right fit, there are other options to compare side by side in Plan Finder.
- If you enroll in a Medicare Advantage plan for January 1 and change your mind, the Medicare Advantage Open Enrollment Period (January 1 – March 31, 2027) lets you switch to a different MA plan or go back to Original Medicare once.
