According to the CMS Medicare Advantage Landscape Source File for 2027, Aetna (which CMS lists as Aetna (CVS Health)) will offer three Medicare Advantage plans in Santa Clara County next year, down from four in 2026. MyMedicarePlans.org is an independent consumer site. We are not Aetna, and we are not Medicare. Aetna is one of ten carriers offering Medicare Advantage plans in Santa Clara County for 2027 — all carriers together offer 36 plans — so the lineup below is one slice of what is available locally.
What Aetna is offering in Santa Clara County for 2027
San Jose is in Santa Clara County, and Medicare Advantage plan availability is set at the county level. For 2027, Aetna's three plans here include one HMO and two HMO-POS plans. There are no Special Needs Plans (D-SNP, C-SNP or I-SNP) from Aetna in the county this year.
The table below lists every Aetna plan offered in Santa Clara County for 2027, with its plan type and drug-coverage status.
What's new, what's renamed, and what's not returning
Three things changed in Aetna's local lineup between 2026 and 2027:
- New for 2027: Aetna Medicare Eagle (HMO) is a new plan in the county.
- Renamed: Aetna Medicare Signature (HMO) is the plan previously sold as Aetna Medicare Enhanced (HMO-POS). Aetna Medicare Signature Extra (HMO) is the plan previously sold as Aetna Medicare Enhanced Extra (HMO-POS).
- Not returning: Two 2026 Aetna plans in Santa Clara County will not be offered in 2027.
A renamed plan is not necessarily the same plan with a new label — benefits, networks and drug lists can shift from year to year. The Annual Notice of Change (ANOC) that your plan mailed in September is the document that lays out exactly what is changing for your 2027 coverage.
If your current plan is not returning, Medicare gives you a Special Enrollment Period to pick something else, but the simplest path is to choose during the regular window that ends December 7.
How to check a plan against your doctors and medications
Before you lock in any plan — Aetna's or another carrier's — do these three checks:
1. Doctors and hospitals. Confirm each provider is in-network for the specific 2027 plan you are considering. Call the provider's billing office directly and name the plan, because networks can change on January 1. 2. Prescriptions. Look up every medication you take on the plan's 2027 formulary. Check the tier, any prior authorization or step therapy, and the preferred pharmacies near you. 3. Total yearly cost. The Medicare Plan Finder at medicare.gov lets you enter your drugs and preferred pharmacies and shows an estimated annual cost for each plan — not just the monthly premium.
2027 Part D figures worth knowing
Even if you keep the same plan, the Part D rules change in 2027. CMS has confirmed:
- The Part D out-of-pocket cap rises to $2,400 in 2027 (up from $2,100 in 2026).
- The Part D maximum deductible rises to $700 in 2027 (up from $615 in 2026).
- The Part D national base beneficiary premium for 2027 is $41.33 — that is a formula input, not the premium any specific plan charges.
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not yet been announced. CMS typically publishes those figures later in the fall.
The coverage gap (the old "donut hole") was eliminated in 2025, so Part D now has three phases: deductible, initial coverage, and catastrophic. If drug costs are a worry, ask your plan about the Medicare Prescription Payment Plan, which spreads out-of-pocket drug costs across the calendar year.
Dates to put on the calendar
- October 15 – December 7, 2026: Annual Enrollment Period. Changes take effect January 1, 2027.
- January 1 – March 31, 2027: Medicare Advantage Open Enrollment. If you are in a Medicare Advantage plan, you get one chance to switch to another MA plan or return to Original Medicare.
Nothing happens to you if you do nothing. Your plan renews with the changes described in its ANOC — which is exactly why it is worth reading the ANOC this fall rather than next January.
