There is also one new HMO I-SNP category (institutional) and new D-SNP-adjacent dialysis plans listed below.
By carrier, the new C-SNPs include Aetna Medicare Chronic Care; Alignment Health Heart & Diabetes Plus 044 and Central Health Harmony Plus 002 (both offered by Alignment Health Plan); Astiva Health C-SNP Bloom and Astiva Health C-SNP Fountain; Champion Compass, Champion Home and Champion Secure; Clever Care Breathe Select+ and Clever Care Total Choice+; Imperial Flex; and SCAN Ansim Care. VillageHealth is adding SCAN DaVita Dialysis Care and SCAN DaVita Dialysis Care Complete, both HMO-POS C-SNPs aimed at people on dialysis.

How is a new Medicare Advantage plan different from a renewing one?
The main difference is history. A plan that renewed from 2026 to 2027 has a prior-year Star Rating, a published formulary that members have used, and a provider network that doctors in Santa Ana have already contracted with.
