What a C-SNP is, and who qualifies

A Chronic Condition Special Needs Plan is a type of Medicare Advantage plan designed for people living with a specific, severe or disabling chronic condition — for example diabetes, chronic heart failure, chronic lung disorders, or end-stage renal disease. Enrollment is limited: to join, you must have the qualifying condition the plan is built around, and your treating provider has to verify that diagnosis for the plan. If the plan can't confirm the condition within the window CMS allows, enrollment can't continue.

C-SNPs are a form of Medicare Advantage, so Original Medicare rules still underlie them, and most include Part D prescription drug coverage. The specifics — what the plan covers, how care is coordinated, which providers are in network, which drugs are on the formulary — vary plan to plan and belong to each plan's own Evidence of Coverage and provider and pharmacy directories.

The statewide count and the carriers offering them

Of Florida's 560 Medicare Advantage plans in 2027, 110 are C-SNPs. For comparison, the state also has 130 Dual Eligible Special Needs Plans (D-SNPs), for people with both Medicare and Medicaid, and 12 Institutional Special Needs Plans (I-SNPs), for people who live in or need an institutional level of care.

The conditions named in plan names

CMS allows C-SNPs to focus on different qualifying chronic conditions, and some Florida plans put the condition right in the plan name. Where a plan name states a condition, that's the condition the plan serves; where a name doesn't, we don't guess.

The Special Enrollment Period for people with a qualifying condition

Most Medicare beneficiaries make choices during the Annual Enrollment Period, which this year runs October 15 – December 7, 2026, with any change taking effect January 1, 2027. People with a qualifying chronic condition have an additional option: once you're diagnosed with a condition a C-SNP serves, you generally have a Special Enrollment Period to join that C-SNP outside the regular fall window. That SEP is tied to your diagnosis and the plan's verification process, so the exact timing depends on your situation. Checking a plan in your county

Because a C-SNP is sold county by county, the statewide count isn't the same as what's available on your street. For 2027, some Florida C-SNPs are offered in a single county while others span 20 or more; the tables on this page show the county counts for each listing, and the Medicare Plan Finder at medicare.gov is where you confirm, by ZIP code, which C-SNPs you can actually enroll in.
