HIALEAH, Fla. — Fall 2026. As Medicare's Annual Enrollment for 2027 coverage opens Oct. 15 and runs through Dec. 7, 2026, residents of Hialeah who live with a serious ongoing illness have a specific slice of the Medicare Advantage market to consider: Chronic Condition Special Needs Plans, or C-SNPs. These plans are built around a qualifying diagnosis, and in Miami-Dade County — the county Hialeah sits in — eight carriers will offer a total of 20 C-SNPs for the 2027 plan year, according to the CMS Medicare Advantage Landscape Source File.

What is a C-SNP and how does it work in Hialeah for 2027?

A C-SNP is a type of Medicare Advantage plan designed for people with a specific severe or disabling chronic condition. It covers everything Original Medicare (Part A and Part B) covers, almost always includes Part D drug coverage, and tailors its provider network, drug list and care coordination around the condition it serves.

In Miami-Dade County, 20 of the 101 Medicare Advantage plans available for 2027 are C-SNPs. The county also has 34 Dual Eligible SNPs (for people with both Medicare and Medicaid) and 6 Institutional SNPs (for people living in nursing homes or similar settings), but those serve different groups.

Who qualifies for a C-SNP, and what paperwork is required?

Enrollment is not open to everyone. You must have the chronic condition the plan is built for, and a doctor or other provider has to confirm the diagnosis to the plan — usually by signing a short verification form after you apply. If the plan cannot verify your condition within the time CMS allows, your enrollment can be cancelled.

CMS lists 15 chronic conditions that can qualify a plan as a C-SNP, including diabetes, chronic heart failure, cardiovascular disorders, chronic lung disorders, and end-stage renal disease. A plan may serve one condition or a group of related ones. Only the plans whose names state a condition (for example, a plan that includes "Diabetes and Heart" or "Lung" in its name) tell you up front which diagnosis they target; for the rest, check the plan's materials or Medicare.gov.

Which carriers offer C-SNPs in Miami-Dade County for 2027?

Eight carriers will offer C-SNPs in the county for 2027. In alphabetical order, with the number of C-SNPs each offers:

The table on this page lists every C-SNP in the county by name and plan type. A couple of notes from the 2026-to-2027 comparison in the CMS Landscape files: Devoted Health's DEVOTED C-SNP ENHANCED 067 FL is a renamed version of its 2026 plan, and two plans — DrExtraCare-SFL from Doctors HealthCare Plans and Gold Heart & Diabetes from Gold Kidney Health Plan — are new C-SNPs for 2027.

When can someone with a chronic condition enroll in a C-SNP?

You do not have to wait for the Oct. 15–Dec. 7, 2026 Annual Enrollment Period. People with a qualifying chronic condition have a Special Enrollment Period (SEP) that lets you join a C-SNP that serves your condition at other times of the year, as long as the plan is accepting members and your diagnosis is verified.

This SEP is one-time per condition for enrolling into a C-SNP; once you are in a C-SNP, your options to switch work under the normal Medicare Advantage rules, including the Medicare Advantage Open Enrollment Period from Jan. 1 through March 31. If you lose eligibility for the C-SNP — for example, if your condition is no longer verified — you get a grace period to move to a different plan.

What else should Hialeah readers know about 2027?

Any Medicare Advantage change you make during Annual Enrollment takes effect Jan. 1, 2027. If you do nothing, your current plan renews for 2027 if it is still offered, with any changes spelled out in the Annual Notice of Change (ANOC) your plan mailed in September. Across all Medicare Advantage plans in Miami-Dade County, 30 plans are not returning for 2027, 16 are new, and 20 have been renamed.

A few program-level figures to keep in mind: for Part D in 2027, the out-of-pocket cap rises to $2,400 (up from $2,100 in 2026) and the maximum deductible rises to $700 (up from $615 in 2026). CMS typically announces the 2027 Part B standard premium, deductible and IRMAA income brackets later in the fall.

For a plan's premium, deductible, copays, drug list, provider network or star rating — none of which we list here — use the Medicare Plan Finder at medicare.gov, call 1-800-MEDICARE (TTY 1-877-486-2048), or read the plan's Evidence of Coverage.

Good to know

Frequently asked questions

Do I need Medicare Parts A and B before I can join a C-SNP in Hialeah?
Yes. A C-SNP is a type of Medicare Advantage plan, so you must have both Part A and Part B and live in the plan's service area. You also need a qualifying chronic condition that the plan can verify with your provider.
What happens if my doctor cannot verify my condition?
If the plan cannot confirm your qualifying diagnosis within the time CMS allows after you enroll, the plan must disenroll you. You would then move back to Original Medicare or into another plan you are eligible for, and you can use the Medicare Advantage Open Enrollment Period (Jan. 1–March 31) or another Special Enrollment Period if one applies.
Can I keep my current doctor if I join a C-SNP?
It depends on the plan's network. C-SNPs in Miami-Dade County are mostly HMOs or HMO-POS plans, which usually require you to use in-network providers except for emergencies. Check each plan's provider directory on Medicare.gov or by calling the plan before you enroll.
If I do nothing during Annual Enrollment, what happens to my coverage on Jan. 1, 2027?
If your current plan is offered again for 2027, you stay in it automatically, with the changes described in your Annual Notice of Change. If your plan is one of the 30 in Miami-Dade County that is not returning, you will receive a notice from the plan and from Medicare explaining your options and timelines.
