JACKSONVILLE, Fla. — If you have a Medicare Advantage plan in Duval County, your mailbox is about to get busy. The Centers for Medicare & Medicaid Services (CMS) has released the 2027 plan lineup, and the picture in Jacksonville looks different than it did this year. Some plans are going away, a new carrier is coming in, and a number of familiar plans have new names.

What is changing in Duval County for 2027

Jacksonville is in Duval County, where Medicare Advantage availability is set at the county level. According to the CMS Medicare Advantage Landscape Source Files comparing 2026 and 2027:

One carrier, Blue Cross Blue Shield (HCSC), will not offer Medicare Advantage plans in Duval County next year. One carrier, Align Senior Care, is entering the county for the first time.

Plans that are leaving after 2026

If you are enrolled in one of these plans, coverage under that specific plan ends December 31, 2026. Your carrier will send a non-renewal notice explaining your options.

Plans leaving Duval County:

If your plan is on this list, you do not have to wait for December 7. When your plan is not renewed, CMS gives you a Special Enrollment Period that runs through the end of February. You can use it to join a different Medicare Advantage plan or to return to Original Medicare and add a stand-alone Part D drug plan.

Fifteen plans are joining the Duval County market for 2027:

Several of these are Special Needs Plans (SNPs), which are limited to people with specific chronic conditions, people who live in an institution, or people who have both Medicare and Medicaid.

Plans that have been renamed

A rename can be confusing when the letter shows up. In most cases the plan is continuing, but under a new name. In Duval County for 2027, 17 plans have been renamed. A few examples from the CMS file:

Even if a plan kept the same carrier and roughly the same idea, the plan type or other details listed in CMS's file may have shifted. The Annual Notice of Change (ANOC) your carrier mails in September is the document that spells out exactly what is different for your plan.

What to do this fall

Your action list is short, but the order matters.

1. Read your ANOC when it arrives. It compares 2026 to 2027 for the plan you are in now. If your plan is being non-renewed, you will get a separate letter. 2. Check that your doctors, hospitals and pharmacies are still in-network for next year. Networks can change even when a plan continues. 3. Check your drug list. The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027, and the maximum Part D deductible is $615 in 2026 and $700 in 2027, so drug coverage details matter. The coverage gap is gone (there are three phases now), and the optional Medicare Prescription Payment Plan lets you spread drug costs across the year. 4. Compare plans on medicare.gov using your ZIP code and medication list. The Plan Finder shows every option in Duval County side by side. 5. Enroll during the right window. The Annual Enrollment Period is October 15 through December 7. If you are already in a Medicare Advantage plan, you also have Medicare Advantage Open Enrollment from January 1 through March 31 to make one switch. If your plan is not renewed, you have a Special Enrollment Period through the end of February.
