MyMedicarePlans.org is an independent news and education site. We are not Humana, and we are not Medicare. Humana is one of 16 carriers selling Medicare Advantage plans in Miami-Dade County next year — 101 plans in all.

How many Humana Medicare Advantage plans are offered in Miami for 2027?

Humana is offering 31 Medicare Advantage plans in Miami-Dade County — the county Miami sits in — for the 2027 plan year. Of those, 18 are HMO plans and 13 are PPO plans, and every one of them includes Part D prescription drug coverage.

The table on this page lists each plan by name and type. Humana is one carrier among 16 operating in the county next year, so the full local Medicare Advantage market has 101 plans in total.

Which Humana plans are new, leaving, or renamed in Miami-Dade County for 2027?

Three Humana plans available in 2026 are not returning for 2027, two plans are new, and five existing plans are being offered under new names. CMS confirms these counts in its 2026 and 2027 Landscape Source Files.

The two additions for 2027 are Humana Dual Integrated (PPO D-SNP) and HumanaChoice Giveback H7617-145 (PPO). The renamed plans — same contract, new label — are:

If you are enrolled in one of the three plans not returning, you will get a non-renewal notice from Humana. If you do nothing by December 7, 2026, you may end up in Original Medicare on January 1 — so this is the fall to pay attention.

What are Humana's Special Needs Plans in Miami for 2027?

Humana is offering 13 Special Needs Plans (SNPs) in Miami-Dade County for 2027: seven Dual Eligible SNPs (D-SNPs) for people who have both Medicare and Medicaid, and six Chronic Condition SNPs (C-SNPs) for people with qualifying conditions such as diabetes, heart conditions, or chronic lung disease. There are no Institutional SNPs on Humana's 2027 Miami roster.

A C-SNP or D-SNP has its own eligibility rules. You can view the Humana SNPs — including CareBreeze, CareComplete, CareNeeds Platinum, Humana Dual Integrated, and Humana Gold Plus - Diabetes and Heart — in the table below, and confirm whether you qualify through medicare.gov or by calling 1-800-MEDICARE.

How do I check a Humana plan against my doctors and prescriptions before December 7, 2026?

The fastest way is the Medicare Plan Finder at medicare.gov: enter your ZIP code, add your prescriptions, and the tool will show you which plans cover each drug and what tier it falls on. You can also call 1-800-MEDICARE around the clock, or ask Humana directly for the plan's Evidence of Coverage and provider directory.

Three things to check for 2027 coverage before December 7, 2026:

What is changing in Medicare drug coverage for 2027?

Two Part D figures are already set by CMS for 2027. The annual out-of-pocket cap on covered prescriptions rises to $2,400 (up from $2,100 in 2026), and the maximum Part D deductible a plan can charge is $700 (up from $615 in 2026). The Part D national base beneficiary premium for 2027 is $41.33 — this is a formula input, not what any specific plan charges.

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not yet been announced. CMS typically publishes those figures in the fall, usually in November. The Medicare Prescription Payment Plan — the option to spread drug costs over monthly bills — is still available.

What happens if I do nothing by December 7, 2026?

If your current Humana plan is being offered again in 2027, it renews automatically on January 1, 2027, with whatever changes are listed in its ANOC. If your plan is one of the three not returning, you need to pick a new plan during open enrollment or you will default to Original Medicare without Part D drug coverage.

You also get a second window — the Medicare Advantage Open Enrollment Period, January 1 through March 31, 2027 — to switch Medicare Advantage plans once or move back to Original Medicare.
