What the plan-type labels mean
You will see three plan-type abbreviations in the roster:
- HMO (Health Maintenance Organization): You use the plan's network of doctors and hospitals and typically need a referral to see a specialist.
- HMO-POS (HMO with a Point-of-Service option): Similar to an HMO, but the plan lets you go outside the network for certain services, usually at higher cost.
- PPO (Preferred Provider Organization): You can see providers outside the network, generally at a higher cost share, and referrals are usually not required.
Some PPO plans are labeled "Regional PPO," meaning they serve a multi-county service area defined by CMS.
Special Needs Plans, and who they are for
Of the 123 plans in Orange County, 64 are Special Needs Plans. SNPs are Medicare Advantage plans built for people who meet specific criteria. You will see three types in the roster:
- D-SNP (Dual Eligible): For people who have both Medicare and Medicaid.

A few 2027 program-level facts worth keeping in mind while you compare:
- The Part D annual out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027.
- The Part D maximum deductible is $615 in 2026 ($700 in 2027).
