LEXINGTON, Ky. — Fall 2026 brings the yearly reshuffle of Medicare Advantage, and the 2027 lineup in Fayette County looks a little different from the one on sale today. Twelve plans are new for 2027, eleven that were sold in 2026 will not be back, and nine kept their coverage but changed their name. Nine carriers are competing for Lexington's Medicare shoppers during the Annual Enrollment Period that runs October 15 through December 7, 2026.

What Medicare Advantage plans are offered in Lexington for 2027?

Lexington sits in Fayette County, where CMS lists nine carriers offering Medicare Advantage for the 2027 plan year: Abilis Health, Aetna (CVS Health), Centene (WellCare), Devoted Health, Elevance Health (Anthem), Essence Healthcare, Humana, Passport Advantage and UnitedHealthcare. The table below lists every plan by carrier and plan type.

The lineup covers the three familiar structures — HMO, HMO‑POS and PPO — plus Special Needs Plans (SNPs) for people who are dually eligible for Medicare and Medicaid, who have certain chronic conditions, or who live in an institutional setting. Humana offers the largest slate of plans in the county, followed by UnitedHealthcare and Centene. Smaller carriers such as Essence Healthcare, Passport Advantage and Abilis Health round out the list.

Because this is a county‑level file, the same plans available in Lexington are available throughout Fayette County.

What Medicare Advantage plans are new in Fayette County for 2027?

Twelve plans are new to Fayette County for 2027. They come from five carriers.

Several of the new names are Chronic Condition SNPs (C‑SNPs) or Dual‑Eligible SNPs (D‑SNPs), which are only open to people who meet those eligibility rules.

Which Lexington Medicare Advantage plans are not returning in 2027?

Eleven Medicare Advantage plans sold in Fayette County in 2026 will not be offered in 2027. If you are enrolled in one of them, your coverage ends December 31, 2026, and you will receive a non‑renewal notice from your plan.

The plans leaving after 2026 are:

A plan non‑renewal triggers a Special Enrollment Period: from December 8, 2026 through the end of February 2027, you can pick another Medicare Advantage plan or move back to Original Medicare and add a stand‑alone Part D drug plan. If you do nothing, Medicare returns you to Original Medicare on January 1, 2027, but you will not automatically have drug coverage.

What happens if my plan was renamed for 2027?

Nine Fayette County plans continue into 2027 under a new name. A rename is not a plan change on its own, but the plan's Annual Notice of Change (ANOC) may show other updates, so read it before deciding.

Renamed plans include Wellcare Dual Access (HMO‑POS D‑SNP), formerly Wellcare Dual Access Sync; DEVOTED C‑SNP CHOICE ENHANCED 006 KY (PPO C‑SNP), formerly DEVOTED C‑SNP CHOICE PREMIUM 006 KY; Humana Essentials Plus Giveback (PPO), formerly HumanaChoice Giveback H7617‑049; Humana Value Choice (PPO), formerly HumanaChoice H7617‑050; HumanaChoice H5216‑226 (PPO), formerly HumanaChoice Giveback H5216‑226; AARP Medicare Advantage from UHC KY‑1 (HMO‑POS), formerly AARP Medicare Advantage Essentials from UHC KY‑1; AARP Medicare Advantage from UHC KY‑5 (HMO‑POS), formerly AARP Medicare Advantage Extras from UHC KY‑5; UHC Dual Advantage KY‑V1 (HMO‑POS D‑SNP), formerly UHC Dual Complete KY‑V001; and UHC Dual Complete KY‑Q1 (PPO D‑SNP), formerly UHC Dual Complete KY‑S001.

If you cannot find your plan on the carrier's new paperwork, check whether it was simply renamed before you assume it was dropped.

When can Lexington residents make changes for 2027?

The Annual Enrollment Period runs October 15 through December 7, 2026. Any change you make — switching Medicare Advantage plans, moving from Original Medicare to Advantage or back, adding or changing Part D drug coverage — takes effect January 1, 2027. If you take no action and your current plan continues, it renews automatically with whatever updates its ANOC describes.

A second window, the Medicare Advantage Open Enrollment Period, runs January 1 through March 31, 2027, for people already in a Medicare Advantage plan who want to switch once or return to Original Medicare.

What program changes should I know about for 2027?

Two Part D figures are set for 2027. The out‑of‑pocket cap on covered prescription drugs rises to $2,400 in 2027 (up from $2,100 in 2026). The maximum Part D deductible a plan can charge is $700 in 2027 (up from $615 in 2026). Once your out‑of‑pocket drug spending hits the cap, your Part D drug costs are $0 for the rest of the year. The Medicare Prescription Payment Plan — an optional way to spread drug costs across the year in monthly bills — is still available.

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not yet been announced. CMS typically publishes them later in the fall.

Good to know

Frequently asked questions

What should I do if my Medicare Advantage plan is on the "leaving" list?
You will get a non‑renewal letter from the plan in the fall. Because the plan is ending, Medicare gives you a Special Enrollment Period from December 8, 2026 through the end of February 2027 to join a different Medicare Advantage plan or return to Original Medicare with a stand‑alone Part D plan. If you make no choice, you will be moved to Original Medicare on January 1, 2027, without drug coverage.
Do I have to do anything during the Annual Enrollment Period if I'm happy with my plan?
No. If your current plan is offered again in 2027, it renews automatically. The Annual Notice of Change your plan mails in September describes any updates to premium, deductible, copays, drug list or provider network, so it is worth reading before you decide to stay put.
How do I compare the 2027 plans available in Lexington?
Use the Medicare Plan Finder at medicare.gov, where you can enter your ZIP code and medications to see every Medicare Advantage and Part D plan sold in Fayette County, with each plan's own premium, cost‑sharing and drug list. You can also call 1‑800‑MEDICARE, which is open 24 hours a day, seven days a week.
What is the difference between an HMO, HMO‑POS and PPO plan?
An HMO uses an in‑network set of doctors and hospitals and usually requires a primary care doctor and referrals. An HMO‑POS (point‑of‑service) is an HMO that allows some out‑of‑network care, typically at a higher cost. A PPO lets you see out‑of‑network providers who accept Medicare, usually at a higher cost than in‑network care, and generally does not require referrals. For plan‑by‑plan details and to enroll, visit medicare.gov or call 1‑800‑MEDICARE (TTY 1‑877‑486‑2048).
