LOUISVILLE, Ky. — Fall 2026 is here, and the Medicare Advantage picture in Jefferson County is shifting more than usual for the plan year ahead. According to the CMS 2027 Medicare Advantage Landscape Source File, 13 plans available in the county this year will not be offered in 2027, 13 brand-new plans are coming in, one carrier is leaving the county entirely, and one carrier new to the county is joining the roster. If you live in Louisville and already have Medicare Advantage, those changes are the reason to open the envelope your plan sends every fall.

That envelope — the Annual Notice of Change, or ANOC — tells you what your current plan will look like on January 1, 2027. The Annual Enrollment Period runs October 15 through December 7, 2026, and any change you make in that window takes effect at the start of the new year.

What the Louisville lineup looks like today

Louisville sits in Jefferson County, and Medicare Advantage availability is set at the county level. For the current (2026) plan year, Jefferson County residents can choose from 67 Medicare Advantage plans sold by 10 carriers: Abilis Health, Aetna (CVS Health), Centene (WellCare), Devoted Health, Elevance Health (Anthem), Essence Healthcare, Humana, Passport Advantage, Provider Partners Health Plans and UnitedHealthcare.

By plan type, the county has 15 HMOs, 17 HMO-POS plans and 35 PPOs. Twenty-eight of the 67 are Special Needs Plans (SNPs) — the kind designed for people with Medicaid as well as Medicare (D-SNP), with a qualifying chronic condition (C-SNP), or who need an institutional level of care (I-SNP). All 67 include prescription drug coverage.

Plans not returning for 2027

Thirteen Jefferson County plans will not be offered in 2027. If one of them is yours, your carrier is required to tell you in writing. The non-renewing plans are:

If your plan is on this list, you are not left without options. Medicare gives you a Special Enrollment Period that runs from December 8, 2026 through the end of February 2027 to pick another Medicare Advantage plan or go back to Original Medicare and add a stand-alone Part D drug plan. You can use the regular Annual Enrollment window first (through December 7) if you want your new choice to start January 1.

Plans new to Louisville for 2027

Thirteen plans are new in Jefferson County for 2027, and they come from carriers already in the market plus one newcomer:

Several of these are Special Needs Plans, aimed at people who have Medicare and Medicaid, who live in a long-term care setting, or who have a specific chronic condition. SNPs have eligibility rules, so a plan being "new" does not mean anyone can join it.

Renames and other changes to watch

Nine Louisville-area plans are coming back in 2027 under a different name. The name change alone does not change what the plan does, but it is easy to miss in paperwork. The renamed plans are: Wellcare Dual Access (HMO-POS D-SNP), formerly Wellcare Dual Access Sync; DEVOTED C-SNP CHOICE ENHANCED 006 KY (PPO C-SNP), formerly DEVOTED C-SNP CHOICE PREMIUM 006 KY; Humana Essentials Plus Giveback (PPO), formerly HumanaChoice Giveback H7617-049; Humana Value Choice (PPO), formerly HumanaChoice H7617-050; HumanaChoice H5216-226 (PPO), formerly HumanaChoice Giveback H5216-226; AARP Medicare Advantage from UHC KY-1 (HMO-POS), formerly AARP Medicare Advantage Essentials from UHC KY-1; AARP Medicare Advantage from UHC KY-5 (HMO-POS), formerly AARP Medicare Advantage Extras from UHC KY-5; UHC Dual Advantage KY-V1 (HMO-POS D-SNP), formerly UHC Dual Complete KY-V001; and UHC Dual Complete KY-Q1 (PPO D-SNP), formerly UHC Dual Complete KY-S001.

In total, CMS data show 14 Louisville-area plans have a change on file for 2027 — in premium, deductible, type or name — and 45 plans continue without a flagged change. The ANOC from each plan is the document that spells out exactly what will be different.

What is changing at the program level in 2027

A few program-wide numbers are already set for next year and apply no matter which plan you pick:

The coverage gap, or "donut hole," was eliminated starting in 2025, so Part D has three phases now. The Medicare Prescription Payment Plan — an option that lets you spread drug costs over the calendar year in monthly bills — is still available for 2027.

A few other 2027 figures are not out yet. CMS typically publishes the standard Part B premium and deductible, and the IRMAA income brackets that affect higher earners, later in the fall. Until then, no reliable 2027 number for those exists.
