For the current (2026) plan year, Milwaukee County residents can choose among plans from Aetna (CVS Health), Centene (WellCare), Elevance Health (Anthem), Humana, Molina Healthcare of Wisconsin, Network Health Medicare Advantage Plans, and UnitedHealthcare. By plan type, the 52 plans break down as 6 HMO, 17 HMO-POS, 27 PPO, 1 PFFS, and 1 MSA. Eighteen of them are Special Needs Plans, and all 52 include drug coverage.

Plans not returning for 2027

Six plans are leaving Milwaukee County after 2026:

If you are enrolled in one of these plans, you will get a non-renewal notice from your carrier. That notice triggers a Special Enrollment Period: you have from December 8, 2026 through the end of February 2027 to join another Medicare Advantage plan or go back to Original Medicare and add a stand-alone Part D plan. You don't need to wait — you can also pick a new plan during the Annual Enrollment Period.

Plans new for 2027

Six plans are being added in Milwaukee County for 2027:

Several of the new entrants are Special Needs Plans — D-SNPs for people with both Medicare and Medicaid, and C-SNPs for people with specific chronic conditions. SNPs have eligibility rules; your plan or 1-800-MEDICARE can confirm whether you qualify.

Renamed plans — same carrier, new label

Twelve plans are returning for 2027 under a new name. A rename is not a cancellation — your coverage continues — but the name on your card and in Medicare Plan Finder will change. Examples include Wellcare Dual Access Sync becoming Wellcare MHS Health Wisconsin Dual Liberty Sync; several Humana plans being relabeled (for instance, Humana Gold Plus H6622-001 is now Humana Total Complete); the former My Choice Wisconsin plans now carrying the Molina name; and several UHC Dual Complete plans renamed as UHC Dual Advantage. If you see an unfamiliar name on your renewal paperwork, check whether it's the same plan under a new title.

What's changing in Part D for 2027

Even if you keep the same plan, the Part D rules are shifting:

CMS has not yet announced the 2027 Part B standard premium and deductible, or the 2027 IRMAA income brackets. Those usually come out in November; check medicare.gov for the official numbers when they publish.

How to use this enrollment season

A few plain steps:

1. Open your ANOC and look for changes to premium, deductible, drug tiers, and your doctors' network status. 2. If your plan is on the "leaving" list above, note your Special Enrollment Period dates. 3. Use the Medicare Plan Finder at medicare.gov to compare plans using your own prescriptions and pharmacies. 4. If you do nothing and your plan is still offered, it renews automatically on January 1, 2027.
