What is new, and what is not returning
CareFirst is adding one plan in Baltimore city for 2027: Advantage Signature (PPO). No CareFirst plans are being discontinued here. Two plans — Advantage Complete (PPO) and Advantage Salute (PPO) — are returning.
Returning does not mean unchanged. Every Medicare Advantage plan sends members an Annual Notice of Change (ANOC) each fall. The ANOC spells out what is different for next year: premium, deductible, copays, drug tiers, extra benefits and provider network. If you are currently on a CareFirst plan, the ANOC that arrived in September is the single most important document to read before December 7.
Special Needs Plans and drug coverage
CareFirst's three Baltimore plans for 2027 all include Part D prescription drug coverage, so you would not need a separate standalone drug plan if you enroll.
CareFirst is not offering any Special Needs Plans in Baltimore city for 2027 — no Dual Eligible SNPs (for people with both Medicare and Medicaid), no Chronic Condition SNPs and no Institutional SNPs. How to check a plan against your doctors and prescriptions
Before December 7, 2026, three checks matter more than anything else:
1. Your doctors and hospitals. On medicare.gov's Plan Finder, open the plan's details and look at the provider directory, or call the plan directly. Confirm each doctor, specialist and hospital you use is in-network for 2027 — networks can change year to year. 2. Your prescriptions. Enter every drug you take, with doses, into the Plan Finder. It will show the plan's formulary tier, any prior authorization, and your projected yearly drug cost. Watch for tier changes between 2026 and 2027. 3. Your pharmacy. "Preferred" versus "standard" pharmacy status affects what you pay. Confirm your pharmacy's status under the plan for 2027.

Dates and program figures to keep in mind
- Annual Enrollment Period: October 15 – December 7, 2026. Any change takes effect January 1, 2027. If you do nothing and your plan is returning, you stay enrolled with the changes in your ANOC.
- Medicare Advantage Open Enrollment: January 1 – March 31, 2027, for people already in a Medicare Advantage plan who want one chance to switch or go back to Original Medicare.
- Part D out-of-pocket cap: $2,100 in 2026, rising to $2,400 in 2027.
- Part D maximum deductible: $615 in 2026, rising to $700 in 2027.
- Part D national base beneficiary premium: $41.33 for 2027 (a formula input used by CMS, not what any specific plan charges).
- 2027 Part B premium, deductible and IRMAA brackets: CMS has not announced these yet. They are usually published in the fall, typically November. Watch medicare.gov.
