Kaiser Permanente's Baltimore lineup for 2027

In Baltimore city, Kaiser Permanente is offering five Medicare Advantage plans for the 2027 plan year. By plan type, that is three HMO plans and two HMO-POS plans. All five include Part D prescription drug coverage.

The table on this page lists every plan by name. In short, the lineup includes two Dual Special Needs Plans (Kaiser Permanente Dual Complete Plan 2 MD and Kaiser Permanente Dual Essential Plan 2 MD), one HMO (Kaiser Permanente Medicare Advantage Liberty), and two HMO-POS plans (Kaiser Permanente Medicare Advantage High MD and Kaiser Permanente Medicare Advantage Standard 2 MD).

We cannot — by rule — publish premiums, deductibles, copays, benefits, provider networks or star ratings here. Those details sit in each plan's Summary of Benefits and Evidence of Coverage, and on medicare.gov.

Special Needs Plans in the mix

Two of Kaiser Permanente's five Baltimore city plans are Dual Special Needs Plans, or D-SNPs. D-SNPs are built for people who qualify for both Medicare and Medicaid. They coordinate benefits between the two programs and generally have different enrollment rules than a standard Medicare Advantage plan.

Kaiser Permanente is not offering a Chronic Condition SNP (C-SNP) or an Institutional SNP (I-SNP) in Baltimore city for 2027. If you think a D-SNP might fit your situation, Maryland Medicaid eligibility is the starting question — call 1-800-MEDICARE or use the Plan Finder to see your options.

What is new, and what is not returning

Baltimore is in Baltimore city, where CMS tracks plan availability. Based on the CMS MA Landscape Source Files for 2026 and 2027, Kaiser Permanente is not adding any new Medicare Advantage plans in Baltimore city for 2027, and none of its current plans are leaving. No plan names are changing either.

That does not mean every detail is the same next year. Costs, drug lists, provider networks and extra benefits can shift from one plan year to the next, and your plan's Annual Notice of Change (ANOC) — mailed in September — is where those specifics live. If you did not keep yours, you can request another copy from the plan.

How to check a plan against your doctors and prescriptions

Before December 7, 2026, take a few minutes to confirm that whichever plan you are considering still fits your care. Here is a clean way to do it:

1. Gather your list. Write down your current doctors and specialists, any hospital you prefer, and every prescription you take — name, dose and how often. 2. Use the Medicare Plan Finder. Go to medicare.gov/plan-compare, enter your ZIP code and your drugs, and the tool will show each plan's drug costs for your specific medications. You can filter to a single carrier. 3. Check the provider directory. Each plan has its own online directory. Search your doctors by name, and confirm the location you actually use is in-network. HMO and HMO-POS plans handle out-of-network care differently, so this matters. 4. Call to double-check. If anything is unclear, call the plan directly, or call 1-800-MEDICARE (TTY 1-877-486-2048), which is open 24/7.

If you do nothing, your current plan simply renews on January 1, 2027 — with whatever changes its ANOC describes — as long as the plan is still offered. In Kaiser Permanente's case in Baltimore city, all five plans are returning.
