Area counts are for the county as a whole and are not a recommendation of any plan. Program figures are set nationally.

St. Paul, Minn. — The 2027 Medicare Advantage lineup for Ramsey County is set, and it looks noticeably different from the one on sale today. Eighteen plans available in 2026 will not return, eight new plans arrive, and one national carrier — Humana — will not offer a Medicare Advantage plan in the county next year. The Annual Enrollment Period that locks in these 2027 choices runs October 15 through December 7, 2026.

What is changing in St. Paul's Medicare Advantage lineup for 2027?

St. Paul sits in Ramsey County, where seven carriers currently offer 37 Medicare Advantage plans for 2026. For 2027, 16 of those plans continue, 18 are leaving, 8 are new, and 3 are being renamed. A handful of continuing plans are also changing on premium, deductible, type or name — the plan's Annual Notice of Change (ANOC), mailed each fall, is where those details live.

The headline shift: Humana is exiting the county. Its five 2026 plans — Humana USAA Honor Giveback (PPO), HumanaChoice H5216-063 (PPO), HumanaChoice H5216-092 (PPO), HumanaChoice H5216-275 (PPO) and HumanaChoice H5216-359 (PPO) — are all on the list of plans not returning for 2027.

Which Medicare Advantage plans are new in Ramsey County for 2027?

Eight plans are new for 2027, from three carriers. Blue Cross and Blue Shield of Minnesota is adding Blue Cross Medicare Advantage Essential (PPO), Blue Cross Medicare Advantage Premier (PPO) and Blue Cross Medicare Advantage Select (PPO). HealthPartners is introducing HealthPartners Journey Pace (PPO) and HealthPartners Journey Smart (PPO). Medica is adding Medica Advantage Preferred (PPO), Medica Advantage Select (PPO) and Medica Advantage Value (PPO).

Three plans are also being renamed rather than discontinued: Allina Health Aetna Medicare Premier (PPO) was Allina Health Aetna Medicare Grand (PPO); Allina Health Aetna Medicare Premier Extra (PPO) was Allina Health Aetna Medicare Grand Extra (PPO); and Medica Advantage Salute (PPO) was Medica Advantage Solution H8889-009 (PPO). The table on this page shows every 2027 plan by carrier and type.

Which Medicare Advantage plans are leaving St. Paul after 2026?

Eighteen plans available in Ramsey County in 2026 will not be offered for 2027. From Blue Cross and Blue Shield of Minnesota: Blue Cross Medicare Advantage Choice (PPO), Blue Cross Medicare Advantage Comfort (PPO), Blue Cross Medicare Advantage Complete (PPO) and Blue Cross Medicare Advantage Core (PPO). From HealthPartners: HealthPartners Journey Pace (PPO), HealthPartners Journey Smart (PPO), HealthPartners Journey Steady (PPO) and HealthPartners Journey Stride (PPO). From Humana: Humana USAA Honor Giveback (PPO), HumanaChoice H5216-063 (PPO), HumanaChoice H5216-092 (PPO), HumanaChoice H5216-275 (PPO) and HumanaChoice H5216-359 (PPO). From Medica: Medica Advantage Solution H6154-001 (HMO-POS), Medica Advantage Solution H8889-001 (PPO), Medica Advantage Solution H8889-003 (PPO) and Medica Advantage Solution H8889-005 (PPO). From UnitedHealthcare: AARP Medicare Advantage from UHC MN-0005 (PPO).

A quick note on the HealthPartners "Journey Pace" and "Journey Smart" names: the 2026 versions are not continuing, and HealthPartners is offering 2027 plans using those same names. Members should read the ANOC closely — a familiar name does not mean a continuing contract.

What happens if my Medicare Advantage plan is not returning in 2027?

If your plan is on the "leaving after 2026" list, your carrier will send a non-renewal notice this fall. You will not be dropped from Medicare — your Part A and Part B coverage continues — but your Medicare Advantage coverage will end on December 31, 2026 unless you pick a new plan.

Two paths to choose from:

Humana members in Ramsey County fall into this second group because Humana will not offer a plan here for 2027.

Who still offers Medicare Advantage in St. Paul for 2027?

Six carriers remain active in Ramsey County for 2027: Aetna (CVS Health), Blue Cross and Blue Shield of Minnesota, Blue Plus, HealthPartners, Medica and UnitedHealthcare. The lineup still includes HMO, HMO-POS and PPO plans, as well as Special Needs Plans for people with certain chronic conditions (C-SNPs) or who have both Medicare and Medicaid (D-SNPs). The table on this page lists every 2027 plan by carrier, plan type and whether it includes drug coverage.

What else is changing for 2027 beyond the plan list?

A couple of program-wide Part D numbers are confirmed for 2027. The Part D out-of-pocket cap rises to $2,400 in 2027, up from $2,100 in 2026. The maximum Part D deductible a plan can charge rises to $700 in 2027, from $615 in 2026. CMS also set the national base beneficiary premium at $41.33 for 2027 — that is a formula input, not what any specific plan charges.

Two important numbers are not out yet: the 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets. CMS typically publishes those in the fall, usually in November. We will update coverage when the figures are released.

Frequently asked questions

Do I have to do anything during the Annual Enrollment Period if I like my current plan?
No. If your 2026 plan is on the "continuing" list for 2027, it renews automatically on January 1, 2027. Read the Annual Notice of Change your carrier sends in September so you know what is changing on premium, deductible, drug list, network and benefits before the window closes on December 7, 2026.
My plan is one of the 18 leaving Ramsey County — when do I need to pick a new one?
You can switch during the Annual Enrollment Period (October 15 – December 7, 2026) for coverage that starts January 1, 2027. If you do not act in that window, Medicare gives you a Special Enrollment Period that generally runs December 8, 2026 through the end of February 2027 to join a different Medicare Advantage plan or move to Original Medicare with a Part D plan.
Are there still Humana options in St. Paul for 2027?
Not for Medicare Advantage in Ramsey County. All five Humana Medicare Advantage plans sold here in 2026 are on the list of plans not returning for 2027. Humana may still sell other Medicare products in Minnesota — Medicare.gov's Plan Finder shows what is available by ZIP code.
Where can I compare the actual 2027 plan details — premiums, drug lists and networks?
The official tool is the Medicare Plan Finder at medicare.gov, which lets you enter your ZIP code and your prescriptions. You can also call 1-800-MEDICARE (TTY 1-877-486-2048), 24 hours a day, seven days a week, or read the Summary of Benefits and Evidence of Coverage posted on each plan's own website.
