Key takeaways

  • Missouri has 174 Medicare Advantage plans listed for 2027 from 12 carriers, but availability is county by county, so compare only the plans offered where you live.
  • All 174 plans include drug coverage, and the mix includes HMO, HMO-POS, PPO and PFFS designs plus Special Needs Plans for specific situations.
  • The Annual Enrollment Period is October 15 through December 7, 2026, and any change you make takes effect January 1, 2027.
  • Use the Medicare Plan Finder at medicare.gov or call 1-800-MEDICARE to confirm doctors, hospitals, prescriptions and pharmacies before you choose.

Because Medicare Advantage is sold county by county, a plan counted here statewide may or may not be offered in your ZIP code. The practical move is to pull up the plans in your own county and compare them side by side.

The statewide numbers for 2027

For plan year 2027, CMS lists 174 Medicare Advantage plans in Missouri from 12 carriers. All 174 include prescription drug coverage. Compared with 2026, 38 plans are new for 2027 and 21 are not returning. Those year-over-year shifts are the reason even current enrollees are encouraged to look again: a plan that worked in 2026 may have changed, or it may not be offered at all in 2027.

The full plan-level list — including which plans appear in which county — comes from the CMS MA Landscape Source File and is rendered in the tables on this page.

Plan types and Special Needs Plans

The 174 Missouri plans break down by type as follows: 72 HMO, 49 HMO-POS, 51 PPO and 2 PFFS. Each type handles networks and referrals differently, so the plan type is one of the first things to check when you compare.

Missouri's 2027 landscape also includes Special Needs Plans (SNPs), which are built for people who meet specific eligibility rules:

If you qualify for a D-SNP, C-SNP or I-SNP, those plans are worth looking at separately because enrollment rules and eligibility differ from general Medicare Advantage plans.

The carriers offering plans in Missouri for 2027

Twelve carriers appear in Missouri's 2027 landscape. In alphabetical order, with the plan counts and plan types each carrier offers somewhere in the state:

Again, these are statewide counts. A carrier that lists 42 plans or 2 plans in Missouri may offer only a few of them — or none — in your county. The carrier table on this page shows the full picture.

A step-by-step way to compare plans in your own county

Comparing on paper is less about picking the "right" carrier and more about matching a plan to the care you actually use. Here is a workable order:

1. Make a short list for yourself: the doctors and hospitals you want to keep using, every prescription you take (name, dose, how often), and the pharmacies that are convenient for you. 2. Open the Medicare Plan Finder at medicare.gov and enter your ZIP code. This filters the 174 statewide plans down to the ones actually offered in your county. 3. For each plan you are considering, check whether your doctors and hospitals are in-network, whether your prescriptions are on the plan's drug list, and whether your pharmacy is a preferred pharmacy for that plan. 4. If you are already in a Medicare Advantage plan, read the Annual Notice of Change (ANOC) your plan mailed in September. It spells out what is changing for 2027 — including whether the plan is still being offered. 5. Compare the plan type (HMO, HMO-POS, PPO, PFFS) and, if you qualify, look at D-SNP, C-SNP or I-SNP options separately. 6. Confirm the fine print directly with the plan's Evidence of Coverage and provider and pharmacy directories before you enroll.

The Plan Finder is the official tool for pulling up premiums, cost-sharing, drug coverage and star ratings for each plan in your county — the kind of detail that must come from the source, not from a summary like this one.

What to do before December 7

The Annual Enrollment Period runs October 15 through December 7, 2026. During that window you can:

Whatever you choose by December 7, 2026 takes effect January 1, 2027. If you are satisfied after reading your ANOC and checking your doctors, drugs and pharmacy, you do not have to do anything — most Medicare Advantage plans renew automatically, as long as the plan is still offered in your county.

Frequently asked questions

How many Medicare Advantage plans are available in Missouri for 2027?
CMS lists 174 Medicare Advantage plans in Missouri for 2027 from 12 carriers. All 174 include prescription drug coverage. Availability is county by county, so the plans offered where you live will be a subset of that statewide total.
When can I change my Medicare Advantage plan for 2027?
The Annual Enrollment Period runs October 15 through December 7, 2026. A choice you make in that window takes effect January 1, 2027.
What if my current plan isn't being offered in 2027?
For 2027, 21 plans are not returning and 38 are new. If your plan is ending, your Annual Notice of Change and a separate non-renewal notice from the plan will explain your options and the special enrollment timing. You can compare replacement plans at medicare.gov or by calling 1-800-MEDICARE.
Do I have to switch plans every year?
No. If your plan is still offered in your county and still fits your doctors, prescriptions and pharmacy after you read the Annual Notice of Change, you can stay in it and it will typically renew automatically on January 1, 2027. For plan-by-plan details and to compare the plans offered in your county, go to medicare.gov or call 1-800-MEDICARE (TTY 1-877-486-2048), available 24 hours a day, 7 days a week.
