RENO, Nev. — Fall 2026. If you live with diabetes, heart disease or another serious long-term condition, Medicare offers a specific kind of Advantage plan built around that diagnosis. For plan year 2027, Washoe County — the county Reno sits in — has seven Chronic Condition Special Needs Plans (C-SNPs) on the shelf, from three carriers. This piece lays out the full list, how a C-SNP actually works, and when you can enroll.
What is a Chronic Condition Special Needs Plan, and how does it work in Reno?
A C-SNP is a Medicare Advantage plan that only enrolls people who have a specific severe or disabling chronic condition on CMS's approved list — for example, diabetes, chronic heart failure or certain lung conditions. Because the plan serves one group of patients, the benefits, drug formulary and care coordination are shaped around that condition.
Like other Medicare Advantage plans, a C-SNP replaces Original Medicare for your Part A and Part B services, and these plans in Washoe County all include Part D prescription drug coverage. You still pay your Part B premium, and the plan sets its own premium, deductibles and copays.
How many C-SNPs are offered in Washoe County for 2027, and by which carriers?
Seven of the 42 Medicare Advantage plans available in Washoe County for 2027 are C-SNPs. Three carriers offer them:
- Alignment Health Plan — 1 C-SNP
- Champion Health Plan — 5 C-SNPs
- Prominence Health Plan — 1 C-SNP
The county also has 17 Dual Eligible Special Needs Plans (D-SNPs) for people with both Medicare and Medicaid, and no Institutional SNPs for 2027.
Which specific C-SNP plans are available in Reno for 2027?
Here are the seven C-SNPs offered in Washoe County for 2027, listed alphabetically by carrier. The table below on this page shows the full plan roster; for cost, network and drug details, check each plan on medicare.gov or in its Evidence of Coverage.
- Alignment Health Heart & Diabetes Plus 006 (HMO C-SNP) — Alignment Health Plan; with drug coverage (renamed from Alignment Health Heart & Diabetes NVPlus)
- Champion Advantage (HMO-POS C-SNP) — Champion Health Plan; with drug coverage
- Champion Care (HMO C-SNP) — Champion Health Plan; with drug coverage
- Champion Choice (HMO C-SNP) — Champion Health Plan; with drug coverage
- Champion Connect (HMO-POS C-SNP) — Champion Health Plan; with drug coverage
- Champion Plus (HMO C-SNP) — Champion Health Plan; with drug coverage
- Prominence Diabetes and Heart (HMO C-SNP) — Prominence Health Plan; with drug coverage (renamed from Prominence Diabetes and Heart Giveback)
Only two of these plan names state the condition they serve (both reference heart disease and diabetes). For every other plan, do not assume which chronic condition it targets — ask the carrier or check the Plan Finder.
How do I qualify for a C-SNP, and who confirms the diagnosis?
To enroll in a C-SNP, you need a qualifying chronic condition verified by your provider. The plan sends a form asking your doctor to confirm the diagnosis in writing; if the confirmation does not come back within a set window (typically about a month after you join), the plan can disenroll you.
You also need to be entitled to Medicare Part A and enrolled in Part B, and you need to live in the plan's service area. Reno residents are inside the Washoe County service area for the plans listed above.
When can I enroll in a C-SNP in Reno?
Having a qualifying chronic condition triggers a Special Enrollment Period (SEP) that lets you join a C-SNP outside the usual windows. In practical terms, once a provider confirms your diagnosis, you generally have a chance to enroll in a C-SNP that serves that condition without waiting for fall.
You can also enroll or switch during the Annual Enrollment Period, October 15 – December 7, 2026, with coverage starting January 1, 2027. If you do nothing, your current plan — C-SNP or otherwise — renews for 2027 if it is still offered, with the changes described in its Annual Notice of Change (ANOC).
Two of the seven C-SNPs on Washoe County's 2027 shelf were renamed from 2026; the plan itself may have other changes too, so read the ANOC that arrives in the mail.
What should I check before choosing a C-SNP?
A few things to put on a short list:
- Does your doctor accept the plan, and are your specialists in network?
- Are your prescriptions on the plan's formulary, and at what tier?
- What are the premium, deductible and copays for the services you use most?
- What extra benefits (if any) does the plan include, and are there rules on how to use them?
For 2027, the Part D out-of-pocket cap rises to $2,400 (up from $2,100 in 2026), and the Part D maximum deductible is $700 (up from $615 in 2026). The 2027 Part B standard premium and the IRMAA income brackets have not yet been announced; CMS typically publishes them later in the fall.
