How do you actually enroll in a C-SNP?
You cannot self-certify. To join a C-SNP, a doctor or other qualified provider has to confirm that you have a qualifying chronic condition on Medicare's list. The plan will send a form (often called a "pre-enrollment verification" or similar) that your provider signs and returns; some plans accept a copy of recent medical records.
