NORTH LAS VEGAS, Nev. — If you live with a long-term condition like diabetes, heart failure, chronic lung disease or end-stage kidney disease, there is a type of Medicare Advantage plan built around that diagnosis — and the 2027 lineup for Clark County, which includes North Las Vegas, is now set. For the plan year starting January 1, 2027, Clark County will have 16 Chronic Condition Special Needs Plans (C-SNPs) from seven carriers, according to the CMS Medicare Advantage Landscape Source File for 2027.
Here is what that means for you during this fall's Annual Enrollment Period, which runs October 15 through December 7, 2026.
What is a C-SNP, and how does it work in North Las Vegas?
A C-SNP is a Medicare Advantage plan that only accepts members who have a specific, serious chronic condition. Like any Medicare Advantage plan, it replaces how Original Medicare pays your bills and almost always includes Part D drug coverage. The difference is that the plan's provider network, drug list and extra benefits are shaped around one group of conditions — for example, diabetes and heart disease, or chronic lung disease.
In Clark County, 16 of the 72 Medicare Advantage plans available for 2027 are C-SNPs. The county also has 16 Dual-Eligible SNPs (for people with both Medicare and Medicaid) and 1 Institutional SNP.
Which carriers offer C-SNPs in Clark County for 2027?
Seven carriers will offer C-SNPs in Clark County for 2027. In alphabetical order:

For every other C-SNP, the plan documents (the Summary of Benefits and Evidence of Coverage) spell out which conditions qualify.
A note on changes: both Alignment Health Heart & Diabetes 004 and Alignment Health Heart & Diabetes Plus 006 are renamed versions of 2026 plans, and Humana is adding a new Humana Gold Plus - Diabetes and Heart (HMO C-SNP) for 2027. If a plan you had this year was renamed, your insurer's Annual Notice of Change (ANOC) will explain what else is different.
How do I know if I qualify for a C-SNP?
To enroll in a C-SNP, you need two things:
1. A qualifying chronic condition from the CMS-approved list. The list includes conditions such as diabetes, chronic heart failure, cardiovascular disorders, chronic lung disorders, end-stage renal disease and several others. Each individual plan picks which of those conditions it serves. 2. Written confirmation of your diagnosis from your provider. The plan will send a short form (sometimes called a "pre-enrollment verification" form) to your doctor, who signs and returns it. Without that confirmation, the plan cannot keep you enrolled.
You also need to live in the plan's service area and have both Medicare Part A and Part B.
When can I join a C-SNP — do I have to wait until December 7?
No. People with a qualifying chronic condition get a Special Enrollment Period (SEP) to join a C-SNP, so you are not locked into the October 15 – December 7, 2026 Annual Enrollment Period.
Under this SEP, you can enroll in a C-SNP once your condition is verified, and the coverage usually starts the first of the following month. There are a few things to know:
- The SEP to join is a one-time use for that enrollment; it does not let you jump between C-SNPs whenever you want.
- If your condition goes into remission or you no longer meet the plan's clinical criteria, you also get an SEP to switch to a different Medicare Advantage plan or go back to Original Medicare.
- Dual-eligible status (Medicare + Medicaid) and Extra Help come with their own, separate SEPs.
If you simply want to compare plans for the January 1, 2027 start date, the regular Annual Enrollment Period is the straightforward route. Doing nothing is also an option — your current plan renews for 2027 if it is still offered, with the changes listed in its ANOC.
What 2027 Medicare numbers should I know while I compare?
A few program-wide figures are already confirmed for 2027 and apply to every Medicare Advantage plan with drug coverage, including C-SNPs:
- The Part D out-of-pocket cap is $2,400 in 2027 (up from $2,100 in 2026).
- The Part D maximum deductible is $700 in 2027 (up from $615 in 2026).
- The Part D national base beneficiary premium for 2027 is $41.33. This is a formula input Medicare uses, not what any specific plan charges.
The standard Part B premium and deductible for 2027, along with the IRMAA income brackets, have not yet been announced. CMS usually publishes those in November. The 2026 IRMAA surcharges, for reference, begin above $109,000 in income for a single filer.
How should I actually shop for a C-SNP in North Las Vegas?

1. Confirm your diagnosis with your provider and ask which conditions it falls under on the CMS chronic-condition list. That tells you which C-SNPs you are eligible for. 2.
