Key takeaways

  • Humana is offering 11 Medicare Advantage plans in Bernalillo County for 2027: four HMOs and seven PPOs, all of them including drug coverage.
  • Three of Humana's 2026 plans are not returning for 2027, no plans are new, and four have been renamed — the Annual Notice of Change in your mailbox explains what shifted for your specific plan.
  • Three of the 2027 Humana plans are Special Needs Plans built around diabetes and heart conditions (C-SNPs); there are no D-SNPs or I-SNPs on Humana's 2027 Bernalillo County list.
  • You have until December 7, 2026 to switch, join or drop a plan for coverage that starts January 1, 2027; doing nothing means your current plan simply renews if it is offered next year.

MyMedicarePlans.org is an independent consumer site and is not Humana. Humana is one of seven carriers offering Medicare Advantage in Bernalillo County for 2027, which together list 36 plans.

What Humana is offering in Albuquerque for 2027

Albuquerque is in Bernalillo County, where Medicare Advantage plan availability is set. For 2027, Humana's lineup in the county breaks down this way:

The full plan names are shown in the table on this page. They include Humana Essentials Plus Giveback (PPO), Humana Full Access (PPO), two Humana Gold Plus — Diabetes and Heart (HMO C-SNP) plans, Humana Gold Plus H0028-019 (HMO), Humana Total Complete (HMO), Humana USAA Honor Giveback (PPO), Humana Value Choice (PPO), HumanaChoice — Diabetes and Heart (PPO C-SNP), HumanaChoice H5216-223 (PPO) and HumanaChoice H7617-068 (PPO).

We can tell you these plans exist in the county; we can't tell you what any of them costs, pays or covers. That part is specific to each plan's Evidence of Coverage.

What's new, what's not returning, what's renamed

Humana is not adding any brand-new Medicare Advantage plans in Bernalillo County for 2027. Three of the carrier's 2026 plans are not being offered again next year, and four plans are returning under new names. If one of the discontinued plans is yours, Humana is required to send a non-renewal letter explaining your options and the special enrollment rights that come with it.

For the four renamed plans, the plan itself is still there — just with a cleaner name. The Annual Notice of Change (ANOC) your plan mailed in late September will spell out any changes to the premium, deductible, drug tiers, provider network or extra benefits for 2027.

How to check a plan against your doctors and prescriptions

Before you decide — whether you are staying put or looking at a different Humana plan — walk through this short checklist for 2027:

1. Pull out your ANOC and read the "changes for next year" section. 2. Make a list of your current doctors, hospitals and pharmacies. 3. Make a list of every prescription you take, with the dose and how often. 4. Go to medicare.gov and open the Plan Finder. Enter your ZIP code and your drugs, then look up each plan you are considering. 5. Check each plan's provider directory to confirm your doctors are in-network for 2027 (networks can change on January 1). 6. If a drug matters to you, check the plan's 2027 formulary for the tier, any prior authorization and the preferred pharmacy.

If you'd rather do this by phone, 1-800-MEDICARE (1-800-633-4227) is open 24/7 and can walk through plans with you.

Dates and dollars to keep in mind

The Annual Enrollment Period runs October 15 through December 7, 2026. Any change you make in that window takes effect January 1, 2027. If you take no action, your current plan renews for 2027 as long as it is still being offered.

A few program-level 2027 figures CMS has confirmed:

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes those in the fall, usually in November.

The coverage gap, or "donut hole," was eliminated in 2025, so Part D now has three phases. If drug costs are hard to spread across the year, the Medicare Prescription Payment Plan lets you pay out-of-pocket drug costs in monthly installments instead of all at once at the pharmacy.

Special Needs Plans in the Humana lineup

Three of Humana's 2027 Bernalillo County plans are C-SNPs built for people living with diabetes or a qualifying heart condition. To enroll in a C-SNP, you generally need a doctor's confirmation of the condition. Humana is not offering D-SNPs (for people with both Medicare and Medicaid) or I-SNPs (for people living in an institutional setting) in the county for 2027. If one of those plan types fits your situation, other carriers in the county may offer them — the Plan Finder will show you.

Frequently asked questions

What happens if my Humana plan is one of the three not returning in 2027?
Humana will send you a non-renewal notice before the end of 2026 explaining that your plan will not be offered next year. You get a Special Enrollment Period to pick a new Medicare Advantage or Part D plan, or to return to Original Medicare. If you do nothing, you will usually be moved to Original Medicare on January 1, 2027, without a stand-alone drug plan — so it is worth acting.
Do I have to do anything if I'm happy with my current Humana plan?
No. If your plan is being offered again for 2027, it renews automatically. Still, read the Annual Notice of Change that arrived in late September — the premium, drug list, provider network or extra benefits can change from one year to the next, even if the plan name does not.
How do I confirm my doctor and my prescriptions are covered next year?
The quickest way is medicare.gov's Plan Finder: enter your ZIP code and your drug list, then open each plan to see the 2027 formulary and estimated drug costs. For doctors and hospitals, use the plan's 2027 provider directory (linked from the plan's page) or call Humana directly to confirm the network for January 1, 2027.
Is Humana the only Medicare Advantage carrier in Albuquerque?
No. In Bernalillo County, seven carriers are offering Medicare Advantage for 2027, with 36 plans in total across all of them. This article covers Humana's slice of that lineup; the Plan Finder shows every option. ## Where to go next For personalized answers about any plan in Bernalillo County, use the official sources: medicare.gov (Plan Finder, formularies and provider directories) and 1-800-MEDICARE (1-800-633-4227, TTY 1-877-486-2048), available 24 hours a day, seven days a week. Humana's member services line, printed on the back of your member ID card, can answer plan-specific questions about your 2027 coverage. The deadline to make a change for coverage starting January 1, 2027 is December 7, 2026.
