Key takeaways
- Lucas County will have 12 Chronic Condition Special Needs Plans for 2027, offered by Aetna (CVS Health), Devoted Health, Humana, UnitedHealthcare and Zing Health.
- To join a C-SNP, you need a qualifying chronic condition confirmed in writing by your provider; the plan verifies the diagnosis before enrollment is final.
- A qualifying diagnosis also opens a Special Enrollment Period, so you don't have to wait for the October 15 – December 7 Annual Enrollment window to join or switch C-SNPs.
- If you do nothing during Annual Enrollment, your current plan renews for 2027 if it is still offered; the plan's Annual Notice of Change explains what's changing.

TOLEDO, Ohio — For older adults in Lucas County living with a serious ongoing illness like diabetes, heart disease or end-stage renal disease, a specific slice of the Medicare Advantage market is built around that diagnosis: Chronic Condition Special Needs Plans, or C-SNPs. For plan year 2027, 12 C-SNPs will be offered in Lucas County, the county Toledo is in, from five carriers, according to the CMS Medicare Advantage Landscape Source File. That lineup — including a few renamed and brand-new options — becomes selectable during the Annual Enrollment Period that runs October 15 through December 7, 2026, with coverage starting January 1, 2027.
What is a C-SNP, and how does it work in Toledo for 2027?
A C-SNP is a type of Medicare Advantage plan that only enrolls people with a specific severe or disabling chronic condition. In Lucas County for 2027, 12 of the 87 Medicare Advantage plans are C-SNPs; the area also has 11 Dual-Eligible SNPs (D-SNPs) and 6 Institutional SNPs (I-SNPs), per the CMS Landscape file.
Like other Medicare Advantage plans, a C-SNP bundles your Part A (hospital) and Part B (medical) coverage, and most C-SNPs include Part D drug coverage. The difference is that the plan's provider network, drug list and extra benefits are designed around one chronic condition — for example, diabetes and cardiovascular disorders, or end-stage renal disease. Care coordination is usually a central feature.
Who qualifies for a C-SNP, and how is the diagnosis confirmed?
Only people with a qualifying chronic condition can enroll in a C-SNP, and the condition has to be verified by a licensed health care provider. When you apply, the plan will ask you (and your doctor) to confirm the diagnosis in writing, typically on a short form. If the provider doesn't verify the condition within a set window, the enrollment is cancelled.
You also need Medicare Part A and Part B and must live in the plan's service area. Each C-SNP sets which specific condition or conditions it serves; the plan's own materials and the Medicare Plan Finder at medicare.gov spell out the exact clinical criteria.
Which C-SNPs are offered in Lucas County for 2027?
Twelve C-SNPs will be sold in Lucas County for 2027, from five carriers. The table below lists every plan available; the C-SNPs, in alphabetical order by carrier, are:
- Aetna (CVS Health): Aetna Medicare Chronic Care (HMO C-SNP); Aetna Medicare Chronic Care Total (HMO C-SNP)
- Devoted Health: DEVOTED C-SNP ENHANCED 017 OH (HMO C-SNP) — renamed (was DEVOTED C-SNP PREMIUM 017 OH); DEVOTED C-SNP GIVEBACK EXTRAS 024 OH (HMO C-SNP) — new for 2027; DEVOTED C-SNP PLUS 016 OH (HMO C-SNP)
- Humana: Humana Gold Plus - Diabetes and Heart (HMO C-SNP)
- UnitedHealthcare: UHC Complete Care OH-18 (HMO-POS C-SNP); UHC Complete Care Support OH-2A (HMO-POS C-SNP) — new for 2027
- Zing Health: Zing Elite Diabetes & Heart OH (HMO C-SNP); Zing Elite Essentials Diabetes & Heart OH (HMO C-SNP) — new for 2027; Zing Select Diabetes & Heart OH (HMO C-SNP); Zing Select Dialysis OH (HMO C-SNP)
Where a plan's name states the condition it serves — for example, "Diabetes and Heart" or "Dialysis" — that's the condition the plan is built around. For plans without a condition in the name, check the plan's own materials for its clinical criteria. All 12 C-SNPs include Part D drug coverage.
When can I enroll in a C-SNP in Toledo?
Two enrollment windows matter. First, the Annual Enrollment Period runs October 15 through December 7, 2026; any change you make in that window takes effect January 1, 2027. If you do nothing, your current plan renews for 2027 if it is still offered, with the changes described in its Annual Notice of Change (ANOC).
Second, a qualifying chronic condition triggers a Special Enrollment Period. That means someone newly diagnosed — or who has lived with a qualifying condition and wants to switch into a C-SNP — generally can enroll outside the October–December window. The SEP continues as long as you remain eligible for the C-SNP. If you lose eligibility (for example, your diagnosis is no longer verified), you'll get a separate SEP to move to another Medicare Advantage plan or back to Original Medicare.
What else is changing for Medicare in 2027?
A few program-level numbers are already set for 2027. The Part D out-of-pocket cap rises to $2,400 in 2027 (it is $2,100 in 2026). The Part D maximum deductible is $700 in 2027 (up from $615 in 2026). CMS also set the Part D national base beneficiary premium at $41.33 for 2027 — that's a formula input, not what any particular plan charges.
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet; CMS publishes those later in the fall, usually in November.
Across the broader Lucas County market, 31 Medicare Advantage plans are not returning for 2027, 19 are new, and 4 are renamed. If your plan is affected, your ANOC and non-renewal notice will explain your options.
How should I compare C-SNPs without ranking them?
Because C-SNPs are tailored to a specific condition, the useful comparisons are clinical and personal: Does the plan's network include your specialists and hospital? Is your medication on the plan's drug list, and at what tier? What care coordination or disease-management support does it provide? What are the plan's rules for referrals and prior authorization?
The Medicare Plan Finder at medicare.gov lets you plug in your ZIP code, your drugs and your providers to see those details side by side. The plan's Evidence of Coverage and provider and pharmacy directories are the authoritative sources once you narrow the list.
