If you are enrolled in a Wellcare plan today, your Annual Notice of Change (ANOC) is the document that spells out what is different for your plan in 2027. The window to switch, drop or add coverage is Medicare's Annual Enrollment Period, October 15 – December 7, 2026, for coverage that starts January 1, 2027.

What Wellcare Medicare Advantage plans are offered in Toledo for 2027?
Toledo is in Lucas County, where Wellcare will offer 4 Medicare Advantage plans for 2027. By CMS plan type, all 4 are HMO D-SNPs, and all 4 include Part D prescription drug coverage. Wellcare's 2027 lineup in the county does not include any C-SNPs (chronic-condition plans) or I-SNPs (institutional plans).
The table below lists every Wellcare plan available in Lucas County for 2027, in alphabetical order. The 2027 plans are:
- Wellcare Buckeye Health Dual Access (HMO D-SNP) — new for 2027
- Wellcare Buckeye Health Dual Reserve (HMO D-SNP) — new for 2027
- Wellcare Buckeye MyCare Ohio Dual Align (HMO D-SNP) — new for 2027
- Wellcare Buckeye MyCare Ohio Dual Align Unity (HMO D-SNP) — renamed (previously Wellcare Buckeye MyCare Ohio Dual Align (HMO D-SNP))
D-SNPs are designed for people who qualify for both Medicare and Medicaid. Eligibility rules apply, so a D-SNP is not an option for every reader.
What is new or not returning in Wellcare's Toledo lineup for 2027?
Compared with 2026, Wellcare has 3 new plans, 1 renamed plan, and 4 plans not returning in Lucas County for 2027. If you are in a plan that is not being offered next year, you will get a non-renewal notice from the carrier, and you can pick a new plan during Annual Enrollment or use a special enrollment period that may apply to your situation.
A renamed plan is not necessarily the same plan — benefits, networks, and formularies can still change year to year. The ANOC is where those specifics live.
How does Wellcare's Toledo lineup compare with other carriers in Lucas County?
In Lucas County, Wellcare is 1 of 14 carriers offering Medicare Advantage plans for 2027, and the county has 87 Medicare Advantage plans in total across all carriers next year. That means readers who do not qualify for a D-SNP, or who want to compare HMO, HMO-POS, PPO or non-SNP options, have other carriers to look at on the Medicare Plan Finder.
We do not rank carriers or plans, and we do not recommend one over another. Which plan fits depends on your doctors, your prescriptions, your budget and your eligibility.
How do I check a Wellcare plan against my doctors and prescriptions before December 7?
Use the official Medicare Plan Finder at medicare.gov. Enter your ZIP code, add your prescriptions and your preferred pharmacy, and the tool will show which plans cover your drugs and at what tier. For doctors and hospitals, check the plan's own online provider directory or call the number on the plan's materials — the Plan Finder does not list every provider for every plan.
If you prefer the phone, call 1-800-MEDICARE (1-800-633-4227), 24 hours a day, seven days a week. For the plan details that cost calculators cannot show — exact benefits, prior authorization rules, referral requirements — the plan's Evidence of Coverage is the source of truth.
What 2027 Medicare figures are already confirmed?
CMS has published a few 2027 numbers that affect Part D drug coverage no matter which plan you choose:
- Part D out-of-pocket cap: $2,400 in 2027 (up from $2,100 in 2026).
- Part D maximum deductible: $700 in 2027 (up from $615 in 2026).
- Part D national base beneficiary premium for 2027: $41.33 (this is a formula input, not what any specific plan charges).
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes them later in the fall, usually in November. Do not rely on projections for those figures.
What happens if I do nothing before December 7, 2026?
If you do nothing and your current plan is being offered again in 2027, you stay in it — with the changes described in your ANOC taking effect January 1, 2027. If your plan is one of the 4 Wellcare plans not returning in Lucas County, your carrier is required to send you a non-renewal letter, and you will need to pick a new plan to avoid a gap in Medicare Advantage coverage (you would otherwise return to Original Medicare).
There is also a Medicare Advantage Open Enrollment Period from January 1 to March 31, 2027, during which people already enrolled in a Medicare Advantage plan can make one change.
