Toledo, Ohio — fall 2026. The 2027 Medicare Advantage picture for Toledo looks noticeably different from this year's. In Lucas County, where Toledo sits, two carriers that sold Medicare Advantage plans in 2026 — MediGold and Paramount Elite Medicare Plans — will not offer plans for 2027, and 31 individual plans from across the market are leaving after this year. At the same time, 19 brand-new plans are coming in. Annual Enrollment for 2027 coverage runs Oct. 15 through Dec. 7, 2026, and this is the window to react.

What Medicare Advantage carriers are offered in Toledo for 2027?

Toledo is in Lucas County, where 16 carriers are offering Medicare Advantage plans for the 2026 plan year. For 2027, two of those — MediGold and Paramount Elite Medicare Plans — are not returning to the county. The remaining 14 carriers continue, including Aetna (CVS Health), CareSource MyCare Ohio, Centene (WellCare), Communicare Advantage, Devoted Health, Elevance Health (Anthem), Humana, Medical Mutual of Ohio, Molina Healthcare of Ohio, Perennial Advantage, SummaCare Medicare Advantage Plans, UnitedHealthcare, Valor Health Plan, and Zing Health. The table below lists every plan by carrier and plan type.

Which Medicare Advantage plans are leaving Toledo after 2026?

Thirty-one plans available in Lucas County in 2026 will not be offered for 2027. The clearest break is at the carrier level: every Mount Carmel MediGold plan (Cash Back, Choice, Glory No RX, No Premium) and every Paramount Elite plan (Enhanced, Preferred, Prime, Standard) ends after this year, because neither carrier is returning to the county. Beyond that, Aetna (CVS Health) is retiring Aetna Medicare Dual Care (HMO D-SNP), Aetna Medicare Partial Dual (HMO D-SNP) and Aetna Medicare Signature (HMO-POS). Centene (WellCare) is dropping Wellcare Assist (HMO-POS), Wellcare Dual Access (HMO-POS D-SNP), Wellcare Dual Reserve (HMO-POS D-SNP) and Wellcare Simple (HMO-POS). Devoted Health is ending DEVOTED DUAL 011 OH (HMO D-SNP) and DEVOTED DUAL PLUS 010 OH (HMO D-SNP). Humana is retiring several plans, including Humana Dual Select H5525-046 (PPO D-SNP), Humana Gold Plus SNP-DE H6622-015 (HMO D-SNP), Humana Gold Plus SNP-DE H6622-087 (HMO D-SNP), two Humana USAA Honor Giveback with Rx (PPO) listings, Humana Value Plus H5525-041 (PPO), HumanaChoice Giveback H5216-309 (PPO) and HumanaChoice Giveback H7617-003 (PPO). Medical Mutual of Ohio is ending MedMutual Advantage Signature (HMO-POS). SummaCare Medicare Advantage Plans is retiring SummaCare Medicare Amber (HMO) (a plan by the same name is being introduced for 2027). UnitedHealthcare is dropping four dual-eligible plans: UHC Dual Complete OH-D002 (HMO-POS D-SNP), UHC Dual Complete OH-D1 (PPO D-SNP), UHC Dual Complete OH-S3 (HMO-POS D-SNP) and UHC Dual Complete OH-V002 (HMO-POS D-SNP).

Which Medicare Advantage plans are new in Toledo for 2027?

Nineteen plans are new to Lucas County for 2027. Centene (WellCare) is adding three: Wellcare Buckeye Health Dual Access (HMO D-SNP), Wellcare Buckeye Health Dual Reserve (HMO D-SNP) and Wellcare Buckeye MyCare Ohio Dual Align (HMO D-SNP). Devoted Health is adding DEVOTED C-SNP GIVEBACK EXTRAS 024 OH (HMO C-SNP) and DEVOTED GIVEBACK EXTRAS 026 OH (HMO). Elevance Health (Anthem) is adding Anthem Dual Advantage Plus (HMO D-SNP) and Anthem I MyCare Ohio Full Dual Advantage (HMO D-SNP). Humana is adding Humana Essentials Plus Giveback (PPO) and HumanaChoice Giveback H5216-481 (PPO). Medical Mutual of Ohio is adding five: MedMutual Advantage Classic (HMO-POS), MedMutual Advantage Elite Enhanced (HMO-POS), MedMutual Advantage Elite Preferred (PPO), MedMutual Advantage Elite Prime (HMO-POS) and MedMutual Advantage Elite Standard (HMO-POS). Molina Healthcare of Ohio is adding Molina Complete Care Connect for MyCare Ohio (HMO D-SNP). SummaCare Medicare Advantage Plans is adding SummaCare Medicare Amber (HMO). UnitedHealthcare is adding AARP Medicare Advantage Patriot No Rx OH-MA2 (HMO-POS) and UHC Complete Care Support OH-2A (HMO-POS C-SNP). Zing Health is adding Zing Elite Essentials Diabetes & Heart OH (HMO C-SNP).

Four 2026 plans are also returning for 2027 under new names: Wellcare Buckeye MyCare Ohio Dual Align (HMO D-SNP) becomes Wellcare Buckeye MyCare Ohio Dual Align Unity; DEVOTED C-SNP PREMIUM 017 OH becomes DEVOTED C-SNP ENHANCED 017 OH; Anthem I MyCare Ohio Full Dual Advantage (HMO D-SNP) becomes Anthem I MyCare Ohio Full Dual Support; and Humana Full Access H5525-042 (PPO) becomes Humana Full Access (PPO). A rename is not a drop — if you are in that plan, your coverage continues under the new name, and the Annual Notice of Change (ANOC) your plan mails each fall explains what else is changing.

What happens if my Medicare Advantage plan is leaving Toledo?

If your current plan is on the "leaving" list, your coverage ends on Dec. 31, 2026. You will receive a non-renewal notice from the plan. That notice triggers a Special Enrollment Period that runs from Dec. 8, 2026 through the end of February 2027, letting you pick a new Medicare Advantage plan or a stand-alone Part D plan, or return to Original Medicare. You do not have to wait for the SEP — you can also make the change during Annual Enrollment, between Oct. 15 and Dec. 7, 2026, so your new coverage starts seamlessly on Jan. 1, 2027. If you do nothing and your plan is not returning, you will be left with Original Medicare only, without drug coverage.

What plan types are available in Lucas County, and how many have drug coverage?

For 2026, Lucas County offers 99 Medicare Advantage plans across 16 carriers: 37 HMO plans, 26 HMO-POS plans and 36 PPO plans. Thirty-three of those are Special Needs Plans (D-SNPs for people with both Medicare and Medicaid, C-SNPs for specific chronic conditions, and I-SNPs for institutional residents). Eighty-eight include Part D prescription drug coverage. The 2027 counts will be published in the full CMS Landscape file; the carrier-level changes above are already reflected in the table on this page.

What is changing in Medicare drug costs for 2027?

Two Part D figures are confirmed for 2027. The annual out-of-pocket cap on covered drugs rises to $2,400 (from $2,100 in 2026), and the maximum Part D deductible a plan can charge is $700 (from $615 in 2026). The national base beneficiary premium — a formula input, not what any one plan charges — is $41.33 for 2027. The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet; CMS typically publishes them in November. The Medicare Prescription Payment Plan, which lets you spread drug costs across the year in monthly installments, continues to be available.

When do I need to make a change for 2027?

Annual Enrollment for 2027 coverage runs Oct. 15 through Dec. 7, 2026. A choice you make in that window takes effect Jan. 1, 2027. If you do nothing and your plan is being offered again next year, it renews automatically with the changes described in its ANOC. If you choose a Medicare Advantage plan and want to switch to a different one — or back to Original Medicare — between Jan. 1 and March 31, 2027, you can use the Medicare Advantage Open Enrollment Period to make one change.

Frequently asked questions

How do I find out if my specific Toledo plan is coming back for 2027?
Watch your mail in September and early October 2026 for the Annual Notice of Change (ANOC) from your plan, or a non-renewal letter if the plan is leaving. You can also compare 2027 plans in your ZIP code at medicare.gov/plan-compare starting Oct. 1, 2026, or call 1-800-MEDICARE.
My plan is on the leaving list. Do I lose coverage on Jan. 1, 2027?
You keep Original Medicare (Part A and Part B) automatically — that does not go away. What ends is the Medicare Advantage plan itself. To avoid a gap in drug or extra-benefit coverage, pick a new plan during Annual Enrollment (by Dec. 7, 2026) or use the Special Enrollment Period that runs through the end of February 2027.
Does a plan being "renamed" mean my benefits change?
Not by itself. A rename just changes the name on paperwork. Any benefit, premium, deductible or network changes will be spelled out in your ANOC. Always read that notice before deciding whether to stay or switch.
Are MediGold and Paramount Elite leaving Medicare entirely?
The CMS data shows that neither carrier is offering Medicare Advantage plans in Lucas County for 2027. That is a county-level fact and does not necessarily speak to other product lines or other counties. If you are in one of their plans, you will receive a non-renewal notice and have a Special Enrollment Period to choose new coverage. For personalized help comparing 2027 options in Toledo, visit medicare.gov or call 1-800-MEDICARE (TTY 1-877-486-2048), available 24 hours a day, seven days a week.
