PORTLAND, Ore. — The 2027 Medicare Advantage lineup for Portland is set, and the shape of the market is shifting. Three carriers that sold Medicare Advantage plans in Multnomah County this year will not offer them for 2027, and a handful of new plans are joining the roster. If you live in or around Portland, the Annual Enrollment Period that runs October 15 through December 7, 2026 is the window to review what is changing before coverage starts January 1, 2027.

What's changing in Multnomah County for 2027

Portland sits in Multnomah County, which is where Medicare sets plan availability. Comparing the CMS Medicare Advantage Landscape files for 2026 and 2027, the county shows 18 plans leaving after 2026, 5 plans new for 2027, 7 renamed plans, and 33 plans continuing. In all, 22 of the plans that are continuing have at least one change to premium, deductible, type or name — the Annual Notice of Change (ANOC) your carrier mailed in September spells out what is different in your plan.

The carriers not returning

Three carriers that currently sell Medicare Advantage in Multnomah County will not have Medicare Advantage plans here next year:

If you are enrolled in any of these plans, your coverage ends December 31, 2026. CMS gives you a Special Enrollment Period that runs from December 8, 2026 through the end of February 2027 to join another Medicare Advantage plan or a stand-alone Part D plan, or to go back to Original Medicare. Acting during the regular Annual Enrollment Period — by December 7 — keeps your coverage seamless on January 1.

Other plans leaving after 2026

Several carriers that are staying in the county are trimming their lineups. Plans leaving after 2026 include Aetna Medicare Eagle (PPO) and Aetna Medicare Signature (PPO) from Aetna (CVS Health); Wellcare Dual Access (HMO-POS D-SNP), Wellcare Dual Reserve (HMO-POS D-SNP), Wellcare Simple (HMO-POS) and Wellcare Simple Open (PPO) from Centene (WellCare); DEVOTED GIVEBACK 004 OR (HMO) from Devoted Health; and AARP Medicare Advantage Essentials from UHC OR-4 (HMO-POS) and AARP Medicare Advantage from UHC OR-0003 (HMO-POS) from UnitedHealthcare.

New Medicare Advantage plans for 2027

Five plans are new in Multnomah County for 2027:

A "new" plan on the CMS list may be a brand-new product or a replacement for one that is leaving — the plan's Summary of Benefits and the Medicare Plan Finder are the right places to compare.

Renamed plans — the coverage can still be different

Seven plans in the county are returning under a new name. Renamed plans this year include Aetna Medicare Elite (HMO) (was Aetna Medicare Elite (HMO-POS)); Wellcare Low Premium Open (PPO) (was Wellcare Giveback Open (PPO)); Wellcare Premium Enhanced Open (PPO) (was Wellcare Low Premium Open (PPO)); Wellcare Trillium Health Plan Dual Select Sync (HMO D-SNP) (was Wellcare Dual Select Sync (HMO-POS D-SNP)); Humana Essentials Plus Giveback (HMO) (was Humana Gold Plus Giveback H1036-323 (HMO)); Humana Total Complete (HMO) (was Humana Gold Plus H1036-153 (HMO)); and Humana Value Choice (PPO) (was HumanaChoice H5216-428 (PPO)).

A few 2027 numbers worth knowing

Even if you keep the same plan, two Part D figures are changing in 2027:

The 2027 Part B standard premium and deductible, along with the 2027 IRMAA income brackets, had not been announced as of early fall 2026. CMS typically publishes them in November. The optional Medicare Prescription Payment Plan, which spreads your drug costs across the calendar year in monthly bills, continues in 2027.

What to do during Annual Enrollment

If you do nothing, your current plan renews for 2027 — assuming it is offered next year — with the changes listed in the ANOC. If your plan is leaving, you need to act, but you have time: the Annual Enrollment Period runs through December 7, 2026, and the non-renewal Special Enrollment Period extends into early 2027.
