C-SNPs are a specialized slice of Medicare Advantage designed around one or more qualifying chronic conditions, and they sit alongside 39 Dual Eligible Special Needs Plans (D-SNPs) and 8 Institutional Special Needs Plans (I-SNPs) in the state. What a C-SNP is and who qualifies

A Chronic Condition Special Needs Plan is a type of Medicare Advantage plan built for people with a specific severe or disabling chronic condition — for example, diabetes, chronic heart failure, or cardiovascular disorders — as defined by the Centers for Medicare & Medicaid Services. To enroll in a C-SNP, a person must have Medicare Part A and Part B, live in the plan's service area, and have a qualifying chronic condition verified by a treating provider. Carriers typically ask the enrollee's doctor to confirm the diagnosis before coverage can begin or continue.

C-SNPs are not open enrollment to the general Medicare population. They are, by design, narrow — care teams, drug formularies and extra benefits are organized around the condition the plan serves. The specific benefits, provider networks and costs vary by plan, and are published in each plan's Evidence of Coverage and in the Medicare Plan Finder at medicare.gov.

The statewide count and the carriers offering them

For 2027, five carriers offer C-SNPs in Pennsylvania: Aetna (CVS Health) with 6 plans, Devoted Health with 12, HealthSpring with 1, Humana with 2, and UnitedHealthcare with 3. The full table on this page lists each plan and the number of counties where it is offered.

Several of these plans are new for 2027. Among the newly offered C-SNPs are additional Aetna Medicare Chronic Care (HMO C-SNP) plans from Aetna (CVS Health), five Devoted Health plans in the "DEVOTED C-SNP GIVEBACK EXTRAS" series (027 PA through 031 PA), two HumanaChoice - Diabetes and Heart (PPO C-SNP) entries, and two UHC Complete Care Support PA-1A (HMO-POS C-SNP) listings from UnitedHealthcare. Devoted Health has also renamed its "PREMIUM" line to "ENHANCED" (for example, DEVOTED C-SNP ENHANCED 016 PA was previously DEVOTED C-SNP PREMIUM 016 PA); the plan identifier is the same, but the display name has changed.

The conditions named in plan names, only as named

Most Pennsylvania C-SNP plan names do not spell out the condition they serve. The exceptions in 2027 are the two HumanaChoice - Diabetes and Heart (PPO C-SNP) listings from Humana, whose names identify diabetes and heart conditions. For every other plan — the Aetna Medicare Chronic Care and Aetna Medicare Chronic Care Value lines from Aetna (CVS Health), the full Devoted Health lineup (ENHANCED, PLUS, and GIVEBACK EXTRAS series), HealthSpring Achieve, and the UHC Complete Care and UHC Complete Care Support plans from UnitedHealthcare — the qualifying condition is defined inside the plan documents, not on the label. Confirm the eligible condition with the carrier or in the Evidence of Coverage before enrolling.

The Special Enrollment Period for people with a qualifying condition

Most Medicare beneficiaries use the Annual Enrollment Period, October 15 – December 7, 2026, for a January 1, 2027 effective date. People with a qualifying chronic condition have an additional path: a Special Enrollment Period (SEP) that allows them to enroll in a C-SNP once, outside of AEP, when they first become eligible for that plan. The SEP generally ends when the person makes a C-SNP enrollment. Rules and documentation requirements for SEPs are described at medicare.gov under Medicare enrollment periods and can be confirmed by calling 1-800-MEDICARE.

If a person loses eligibility — for example, if the qualifying condition can no longer be verified — the plan is required to offer a grace period before disenrolling the member. The details are in the plan's materials.
