How to read the plan-type labels
You'll see three main labels in the table:
- HMO (Health Maintenance Organization): You generally pick a primary care doctor and stay in the plan's network, except for emergencies.
- HMO-POS (Point of Service): An HMO with a limited option to see some out-of-network providers, usually at a higher cost.
- PPO (Preferred Provider Organization): More flexibility to see out-of-network doctors, typically at a higher cost-share.
In Chattanooga's 2026 lineup, the mix is 24 HMOs, 19 HMO-POS plans and 22 PPOs.
Special Needs Plans in the mix
Of the 65 plans, 26 are Special Needs Plans (SNPs). These are Advantage plans limited to people who fit a specific situation:
- D-SNP (Dual Eligible): For people who have both Medicare and Medicaid.
- C-SNP (Chronic Condition): For people with a qualifying chronic condition, such as diabetes or certain heart conditions.
- I-SNP (Institutional): For people who live in a nursing home or need a similar level of care.
If you don't meet the eligibility rules for an SNP, you can't enroll in one, even if it's listed for your county. Regular HMO, HMO-POS and PPO plans in the roster are open to any Medicare-eligible resident of Hamilton County.
Drug coverage and the 2027 Part D rules
All 58 Chattanooga-area Advantage plans include Part D drug coverage. A few program-wide 2027 rules apply no matter which plan you pick:
- The Part D out-of-pocket cap is $2,100 for 2026. Once your covered drug spending hits that amount, you pay $0 for covered drugs the rest of the year.
- The maximum Part D deductible allowed is $615 in 2026 ($700 in 2027). Plans can charge less.
- The old coverage gap ("donut hole") was eliminated in 2025. Part D now has three phases: deductible, initial coverage and catastrophic.
- The Medicare Prescription Payment Plan lets you spread your drug costs into monthly payments across the year at no extra charge. You have to opt in.
- IRMAA — the extra premium surcharge for higher earners — starts above $109,000 in income for a single filer in 2026.

- October 15 – December 7: Annual Enrollment Period. Anyone with Medicare can join, switch or drop an Advantage or Part D plan.
- January 1 – March 31: Medicare Advantage Open Enrollment. If you're already in an Advantage plan, you can switch to another one or go back to Original Medicare (and add a Part D plan).
Special Enrollment Periods may also apply if you move, lose other coverage or qualify for Medicaid.
