NASHVILLE — As the Annual Enrollment Period opens for 2027 coverage, the Medicare Advantage menu in and around Nashville is shifting more than it has in a few years. Nashville sits in Davidson County, where CMS records show nine plans arriving for 2027, fourteen plans leaving after 2026, and one familiar carrier — Centene (WellCare) — pulling out of the local market altogether.

What the 2027 lineup looks like in Davidson County

For the current (2026) plan year, Davidson County residents have 74 Medicare Advantage plans from 13 carriers to choose from — 30 HMOs, 18 HMO-POS plans, and 26 PPOs. Thirty of those are Special Needs Plans (SNPs) built for people with specific conditions, dual Medicare-Medicaid eligibility, or who live in an institution. The plans not returning after 2026

Fourteen plans sold in Davidson County this year will not be offered in 2027:

Centene (WellCare) is leaving Davidson County entirely for 2027. If you are in one of these plans, your carrier is required to send you a non-renewal notice. You then qualify for a Special Enrollment Period to pick a new Medicare Advantage plan or return to Original Medicare with a stand-alone Part D plan. That SEP typically runs from December 8 through the end of February — but acting during the regular October 15 – December 7 window keeps your coverage uninterrupted on January 1.

The plans new for 2027

CMS lists nine plans joining the Davidson County market for 2027:

Most of the new entries are PPOs or Special Needs Plans. If you qualify for an SNP — because you have a chronic condition, Medicaid, or live in a long-term care setting — the SNP side of the market is wider for 2027 than many people realize.

Renamed plans aren't new plans

Nine plans available in Davidson County kept their contract and benefits structure but changed names for 2027. A few examples: Aetna Medicare Eagle (PPO) was Aetna Medicare Eagle Giveback (PPO); Wellpoint Dual Advantage Plus (HMO D-SNP) was Wellpoint Full Dual Advantage (HMO D-SNP); and Humana Dual QMB Only (HMO D-SNP) was Humana Gold Plus SNP-DE H4461-038 (HMO D-SNP). A rename is not the same as a non-renewal — your coverage continues, but the plan may still have other changes listed in its ANOC.

The 2027 Part B standard premium and deductible, along with the 2027 IRMAA income brackets, are typically announced by CMS in November. We will update this piece as soon as they publish.

The coverage gap (the old "donut hole") has been eliminated since 2025, and the Medicare Prescription Payment Plan — which lets you spread drug costs across the plan year in monthly payments — remains available if you want to use it.

Three things are worth doing before the deadline:

1. Read your plan's Annual Notice of Change. It will tell you whether your plan is renewing, being renamed, or leaving — and what is changing on premium, deductible, drug list, or network. 2.
