Key takeaways

  • Aetna lists 52 Medicare Advantage plans in Texas for 2027, including 31 HMOs and 21 PPOs, and all 52 include drug coverage.
  • The lineup includes 13 D-SNPs and 8 C-SNPs for people with specific eligibility, plus 10 plans that are new for 2027 and 2 that are not returning.
  • A plan listed statewide may not be offered in your county, so always confirm availability on Medicare.gov before you decide.
  • The Annual Enrollment Period runs October 15 through December 7, 2026, and a choice made in that window takes effect January 1, 2027.

Because Medicare Advantage is sold county by county, any plan named here as "statewide" may not be sold in the county where you live. The plan tables elsewhere on this page show the county counts plan by plan.

What Aetna offers statewide for 2027

For the 2027 plan year, Aetna's Texas Medicare Advantage menu totals 52 plans. By plan type, that breaks down into 31 HMOs and 21 PPOs. Every one of the 52 plans includes Part D prescription drug coverage, so Texans picking an Aetna Medicare Advantage plan would not need a separate stand-alone drug plan.

Aetna also offers Special Needs Plans in Texas for 2027: 13 Dual Eligible Special Needs Plans (D-SNPs) for people who qualify for both Medicare and Medicaid, and 8 Chronic Condition Special Needs Plans (C-SNPs) for people with certain qualifying chronic conditions. Aetna does not list any Institutional Special Needs Plans (I-SNPs) in Texas for 2027.

For context, CMS counts 21 carriers offering Medicare Advantage in Texas for 2027, with 421 plans statewide across all carriers combined.

Plans with the widest county reach

The Aetna plan with the broadest county footprint listed for 2027 is Aetna Medicare Full Dual Care (HMO D-SNP), which appears in 100 counties under one plan ID, along with another new D-SNP, Aetna Medicare QMB Only Care (HMO D-SNP), also listed in 100 counties. Aetna Medicare Full Dual Extra Care (HMO D-SNP) and Aetna Medicare QMB Only Extra Care (HMO D-SNP) each reach 98 counties under their widest listings. Because these are D-SNPs, enrollment is limited to people who meet the dual-eligibility rules.

On the non-SNP side, Aetna Medicare Eagle (PPO) has the widest reach at 48 counties, followed by Aetna Medicare Signature (PPO) at 41 counties under its widest listing. Other broadly offered options include Aetna Medicare Signature Extra (PPO) and Aetna Medicare Value Plus (PPO). The complete plan-by-plan county counts appear in the tables on this page.

What is new or not returning

Aetna has 10 plans that are new for 2027 in Texas. Among them are Aetna Medicare Enhanced Extra (PPO), Aetna Medicare Signature (HMO) in its newly listed service area, several listings of Aetna Medicare QMB Only Care (HMO D-SNP), and additional Aetna Medicare Chronic Care and Chronic Care Total (HMO C-SNP) listings.

A number of 2026 plans are returning under new names. For example, CMS shows Aetna Medicare Dual Care (HMO D-SNP) renamed to Aetna Medicare Full Dual Care (HMO D-SNP); Aetna Medicare Full Dual Care (HMO D-SNP) renamed to Aetna Medicare Full Dual Extra Care (HMO D-SNP); Aetna Medicare Partial Dual Care (HMO D-SNP) renamed to Aetna Medicare QMB Only Extra Care (HMO D-SNP); Aetna Medicare Prime Chronic Care (HMO C-SNP) renamed to Aetna Medicare Chronic Care (HMO C-SNP); Aetna Medicare Prime Chronic Total (HMO C-SNP) renamed to Aetna Medicare Chronic Care Total (HMO C-SNP); Aetna Medicare Prime (HMO) renamed to Aetna Medicare Signature Extra (HMO); and several Aetna Medicare Signature (PPO) listings renamed to Aetna Medicare Signature Extra (PPO). A rename can also come with changes to the plan itself, so current members should read the Annual Notice of Change that arrives in the mail.

Aetna also has 2 Texas plans that are not returning for 2027. If you are enrolled in one, your mailed notice will explain your options.

Checking your county, doctors and prescriptions

A statewide total is a useful snapshot, but Medicare Advantage is bought one ZIP code at a time. To see whether a specific Aetna plan is actually offered where you live:

1. Go to Medicare.gov and use the Medicare Plan Finder with your ZIP code. 2. Filter by Medicare Advantage to see every plan available in your county, from Aetna and from other carriers. 3. Open the plan page to review the premium, deductible, copays, extra benefits and star rating for that specific plan. 4. Enter your prescriptions to see how each plan's drug list treats them. 5. Check the plan's provider directory to confirm that your doctors, specialists and preferred hospitals are in network.

You can also call 1-800-MEDICARE (1-800-633-4227), available 24 hours a day, 7 days a week, or read the plan's own Evidence of Coverage and provider and pharmacy directories.

What to do before December 7

The Annual Enrollment Period for 2027 coverage runs October 15 through December 7, 2026. Any Medicare Advantage or Part D choice you make during that window takes effect January 1, 2027. A simple checklist:
