What a C-SNP is, and who qualifies
A Chronic Condition Special Needs Plan is a type of Medicare Advantage plan designed for people with a specific, severe or disabling chronic condition. To enroll, you need Medicare Part A and Part B, you must live in the plan's service area, and a provider has to verify that you have the qualifying condition the plan covers. If that verification doesn't come through within the window the plan allows, you can't stay enrolled.
C-SNPs include Part D prescription drug coverage. Everything else — the provider network, the specific benefits, the drug list, cost sharing — is set by the individual plan and should be checked in that plan's own documents or on Medicare.gov before enrolling.

The conditions named in the plan names
Medicare rules let a C-SNP serve one qualifying condition or a defined group of conditions. Some Texas plans name the condition right in the plan title; others use a more general "Chronic Care" label and leave the specifics to the plan documents. We'll only tell you a condition when the plan name itself states it.
Plan names that identify a condition for 2027 include:

- Diabetes and heart: Alignment Health Heart & Diabetes 002 and Alignment Health Heart & Diabetes Plus 004 (HMO-POS C-SNPs); Humana Gold Plus - Diabetes and Heart (HMO C-SNP); Prominence Diabetes and Heart Care Plus and Prominence Diabetes and Heart Giveback (HMO C-SNPs).
- Diabetes: ProCare Advantage - Diabetes Care Management (HMO-POS C-SNP).
- Lung: Wellpoint Lung Care Select (HMO-POS C-SNP) from Elevance Health (Anthem).
Plans with names such as Aetna Medicare Chronic Care, Wellpoint Chronic Care, DEVOTED C-SNP, HealthSpring Achieve, MedCare Choice, MedCare Focus, SCAN Balance Texas, SCAN Walmart Medicare Complete, UHC Complete Care, and Verda Noble Chronic Care do not spell out the qualifying condition in the plan name. The Special Enrollment Period for people with a qualifying condition
Most Medicare beneficiaries use the Annual Enrollment Period, October 15 – December 7, 2026, with coverage effective January 1, 2027. People with a qualifying chronic condition have an additional option: a Special Enrollment Period (SEP) that lets them enroll in a C-SNP they're eligible for at other times of the year, once CMS confirms the condition. This SEP can generally be used once per enrollment into a chronic-condition SNP. Medicare.gov and 1-800-MEDICARE can walk through whether the SEP applies to your situation.
Checking a plan in your county
The 67 figure is a statewide count. A plan on this list may serve as few as a single Texas county or stretch across much of the state. The only reliable way to see what's actually available where you live is to check your ZIP code.
Three sources will give you plan-level detail:
1. The Medicare Plan Finder at medicare.gov, which lets you enter your ZIP code, your drugs and your preferred providers. 2. 1-800-MEDICARE (TTY 1-877-486-2048), available 24 hours a day, 7 days a week. 3.
